CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY
WHEREAS, the Indian River County Senior Resource Center has requested authorization to provide Wheelchair Transportation services
that originate within Indian River County;
WHEREAS, the above namedservice affirms that it will maintain compliance with the requirements of the Indian River Code of ordinances
(Chapter 304 , Life Support and Wheelchair Services) .
THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class "E" Certificate of Public Convenience
and Necessity to said Company to provide Wheelchair Transportation services within Indian River County.

Date issued : October 2 , 2007
Date of expiration: October 2 , 2009 i

Limitations: Wheelchair transportation services only. 4
Maintain Contract with Community Transportation Coordinator.
