CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY -
WHEREAS , Indian River Memorial Hospital Medical Transport Service has requested authorization to provide ALS/BLS No
Emergency Interfacility Transport services to the citizens of Indian River County; and n
WHEREAS , a need has been demonstrated to provide these essential services to the citizens of Indian River County; and
WHEREAS , the above named service affirms that it will maintain compliance with the requirements of the Emergency Medical Act, Chapter
401 , Florida Statutes ; Florida Administrative Code, Chapter 64E=2 and Chapter 304, Indian River County Code of Laws and Ordinances ,
THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class ` B " Certificate of Public Convenience
and Necessity to the Indian River Memorial Hospital Medical Transport Service to provide ALS/BLS Non -Emergent Interfacility
Transport Services with limitations as prescribed on this certificate ,
