Council of Indian River County, Inc. has requested authorization to provide Wheelchair Transportation services that originate within Indian River County; WHEREAS, the above named service affirms that if will maintain compliance with the requirements of the Indian River Code of ordinances (Chapter 304 , Life Support and Wheelchair Services) . THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class, "E " Certificate of Public Convenience and Necessity to said Company to provide Wheelchair Transportation services within Indian River County . Certificate Type : Class E Date issued : November 6 , 2005 �V C Date of expiration: November 6 , 2007 (unless certificate is sooner revoked or suspended) Limitations : Wheelchair transportation services only . Maintain Contract with Community Transportation Coordinator, Thomas S .
