CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY
WHEREAS, the Veteran ' s Council of Indian River County, Inc. has requested authorization to provide Wheelchair
Transportation services that originate within Indian River County;
WHEREAS , the above named service affirms that it will maintain compliance with the requirements of the Indian River
County Code of ordinances (Chapter 304, Life Support and Wheelchair Services) .
THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class "E" Certificate of
Public Convenience and Necessity to said Company to provide Wheelchair Transportation, services within Indian River
County .

Date of expiration : November 6 , 2005
Date issued : November 6 , 2003 A (unless certificate is soonei revoked or suspended )
Limitations :
Wheelchair transportation services only.
