WHEREAS, Abilities Resource Center of Indian River County has requested authorization to provide Wheelchair Transportation services
that originate within Indian River County;
WHEREAS, the above named service affirms that it will maintain compliance with the requirements of the Indian River Code of ordinances 11
(Chapter 304 , Life Support and Wheelchair Services) .
THEREFORE, the Board of County Commissioners of Indian River County hereby issues a Class "E" Certificate of Public Convenience ,
and Necessity to said Company to provide Wheelchair Transportation services within Indian River County .
Certificate Type: Class E
Date issued : October 2 , 2007

(unless certificate is sooner revoked or suspended) '
Limitations: Wheelchair transportation services only .
