WORK ORDER NUMBER CT -2
This Work Order Number CT -2 is entered into as of this 14th day of February
2008 pursuant to that certain Continuing Contract Agreement for Professional Services entered
into as of February 19, 2008 ("Agreement"), by and between Indian River County, a political
subdivision of the State of Florida ("COUNTY") and Coastal Technolooy Corporation
("CONSULTANT').
The COUNTY has selected the Consultant to perform the professional services set forth
on Attachment 1, attached to this Work Order and made part hereof by this reference. The
professional services will be performed by the Consultant for the fee schedule set forth in
Attachment 2, attached to this Work Order and made a part hereof by this reference. The
Consultant will perform the professional services within the timeframe more particularly set forth
in Attachment 3, attached to this Work Order and made a part hereof by this reference all in
accordance with the terms and provisions set forth in the Agreement. Pursuant to paragraph 1.4
of the Agreement, nothing contained in any Work Order shall conflict with the terms of the
Agreement and the terms of the Agreement shall be deemed to be incorporated in each
individual Work Order as if fully set forth herein.
IN WITNESS WHEREOF, the parties hereto have executed this Work Order as of the
date first written above.

Attachment 1 TO WORK ORDER CT -2 for Professional Services
WABASSO BEACH PARK RESTORATION
MASTER LIFT STATION AND FORCE MAIN CONNECTION
INTRODUCTION
During the Board of County Commissioners meeting of June 13, 2006 the Board approved Work Order CT -
1 for the restoration of Wabasso Beach Park. The scope of that Work Order included the design of the site
lift station "based upon services to the park restroom facility". This Work Order, CT -2, requires the
CONSULTANT to redesign the lift station included in Work Order CT -1. This Scope of Work entails design
services to provide a master lift station at a location and as required by the Indian River County Utilities
Department. This master lift station will serve the Wabasso Beach Park facilities and be capable of serving
a minimum of approximately one quarter mile gravity sewer main with discharge connection to the existing
force main system in State Road A -1-A, adjacent to the lift station site.
Services are proposed under
the Continuing
Contract Agreement for Professional Services between
COUNTY and CONSULTANT,
dated December 13,
2005. CONSULTANT shall sub -contract water and
sewer related services to Carter
Associates, Inc.
(CAI).
The following describes the work.
SCOPE
ITEM I -PROJECT ADMINISTRATION
A. Project Management. Provide project management and quality control during design of the
project.
B. Administration. Provide services such as: contracting, filing, mailing, reproduction, and
deliveries.
C. Project Meeting. Meet with IRCDUS Operating and Engineering staff to obtain variances
from County Specifications needed to construct proposed facility at desired location.
ITEM II -DESIGN
A. Pre -Design Site Visit. Visit the site to determine additional requirements or conflicts
anticipated with installation of a "Master" lift station. Take digital pictures of existing
conditions. Identify possible site limitations and easement requirements.
B. Master Lift Station Design. Determine "Master" lift station service area based on IRCDUS
requested design criteria. Prepare project calculations and design report for replacing the
proposed private lift station with the requested "Master" lift station. Revise lift station design
template for a caisson installation rather than open hole.
C. Cost Estimate. Prepare a unitized cost estimate for the "Master" lift station installation.

ITEM III — CONSTRUCTION PLANS AND SPECIFICATIONS
A. Master Lift Station. Prepare plan for "Master lift station. Prepare "Master' lift station detail
sheets with revised wet well for caisson construction.
B. "Off -Site" Sewer. Revise profile drawing of "Master' lift station and force main.
C. Master Lift Station Specifications. Revise submersible pump station specification from
private lift station design to comply with "Master" lift station design.
DELIVERABLES
A. Preliminary design
(shall be considered 60
percent design review
submittal) - provide one
copy each of structural and civil construction
drawings together with
one copy of the technical
specifications. Also
provide quality control comments on the 60 percent
review submittal.
B. Final design (shall be 100 percent review submittal) - provide one copy each of all drawings
and technical specifications compatible with EJCDC 1996 General Conditions and provide digital
AutoCAD drawing files 2005 edition or later and digital word document files of the technical
specifications. Also provide one copy of the itemized opinion of probable construction cost.
C. Conformed sets - After the project is bid but before the COUNTY awards the construction
contract, provide five signed and sealed copies of the conformed contract drawings and five
signed and sealed sets of the confirmed technical specifications. Further any revisions to
the above digital drawing and document files shall be provided.

WABASSO BEACH PARK RESTORATION
MASTER LIFT STATION AND FORCE MAIN CONNECTION
1. COMPENSATION
The COUNTY agrees to pay and the CONSULTANT agrees to accept, a not -to -exceed fee of
$7,310.00 for services rendered according to Attachment 1 and the attached FEE SCHEDULE
of this WORK ORDER.
2. PARTIAL PAYMENTS
The COUNTY shall make monthly partial payments to the CONSULTANT for all
authorized work completed per the FEE SCHEDULE and pertaining directly to this project
performed during the previous calendar month. The CONSULTANT shall submit invoices
monthly for services performed and expenses incurred pursuant to this Agreement during the
prior month.
The CONSULTANT shall submit duly certified invoices in duplicate to the Director of the
Public Works Department. For lump sum line items, the amount submitted shall be the prorated
amount due for all work performed to date under this phase, determined by applying the
percentage of the work completed as certified by the CONSULTANT, to the total due for this
phase of the work. The amount of the partial payment due the CONSULTANT for the work
performed to date under these phases shall be an amount calculated in accordance with the
previous paragraph, less ten percent (10%) of the invoice amount thus determined, which shall
be withheld by the COUNTY as retainage, and less previous payments. Per F.S. 218.74(2), the
COUNTY will pay approved invoices on or before the forty-fifth day after the COUNTY receives
the CONSULTANT's invoice.
Per F.S. 218.74(2), the ten percent (10%) retainage withheld shall be paid in full to the
CONSULTANT by the COUNTY, on or before the forty-fifth day after the date of final acceptance
of the Work by the Public Works Director.

TIME FRAME:
The proposed Time Frame is based upon the above stated scope of work and will be
completed within 2 weeks from the date of Notice -to -Proceed as issued by the COUNTY,
unless the scope of work is modified and/or changed as directed by the COUNTY in
writing.
