In accordance with Contract No. 0725 This Work Order No. 1 is in accordance with the existing AGREEMENT dated November 6, 2007 between Creech Engineers, Inc. (SURVEYOR) and Indian River County (COUNTY). This Work Order No. 1 amends the agreement as follows: SECTION I — PROJECT LIMITS This Work Order No. 1 is for the SURVEYOR to perform additional services related to the Indian River County Horizontal Control (GPS) Network (IRCHCN). SECTION II - SCOPE OF SERVICES As agreed upon between Creech Engineers, Inc. and Indian shall provide Professional Land Surveying "Geodetic" services office services to include the two new Indian River County System (CORS) GPS fixed antennas in the mapping and "Blu in the previously contracted project on September 51", 2007 0390). February 29'", 2008 SECTION IV —COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as identified in Sections I, II, and III of this Work Order No. 1, on a time and material, Not—To- Exceed sum of $ 5,000.00 All and/or any additional services not described hereon shall be pre -approved by the COUNTY. Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the contract year(s) of November 6`h, 2007 through November 5ih, 2009. All invoicing shall include project identification, Work Order No. 1 (WO 1), Contract Number 0725, itemize man-hours and materials expended to complete the scope of services. Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services Agreement, contract No: 0725 with the COUNTY and as stated in Section Il, III and IV hereon. The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein. (hereinafter referred to as "SURVEYOR") is pleased to submit this proposal for professional surveying services to INDIAN RIVER COUNTY (hereinafter referred to as "COUNTY") for the GPS Densification Project 2007 located in Indian River County, Florida (hereinafter referred to as "PROJECT"). It is the SURVEYOR'S understanding that the COUNTY is adding to, and strengthening the existing published geodetic control in and around Indian River County for surveying and GIS purposes. To accomplish this task, the COUNTY requested professional surveying and mapping services to Blue Book two (2) Indian River County continuous operating Antenna Base Stations. The two (2) base stations will be included within the Geodetic Control Densification Network we are currently preparing to be submitted to the National Geodetic Survey (NGS) for inclusion into the National Spatial Reference System (NSRS). Scope of Services Task 01 - Continuous Operating Base Station Blue Booking The SURVEYOR will include the two (2) existing Continuous Operating Base Stations within the proposed 2007 Geodetic Control Network to be submitted to the National Geodetic Survey (NGS) for Blue Booking purposes. The SURVEYOR will take digital pictures, prepare "Go To" descriptions, and submit additional information provided by the COUNTY to NGS as a part of the proposed 2007 Geodetic Control Network. The two (2) Continuous Operating Base Stations positions will be published on the 2007 NGS adjustment. Said position will be based upon the bracket or threaded bolt that holds the antenna. February 291", 2008 INFORMATION AND /OR SERVICES PROVIDED BY THE COUNTY The following information and/or services shall be provided to the SURVEYOR by the COUNTY or his representative: A. Specifications and schematics of the brackets, threaded bolts, antenna model, and wiring for both stations; B. Antenna Reference point: The physical location of the bracket or threaded bolt; C. Contact information: The contact information, address, and phone number to be supplied to NGS for inclusion within the "Go To" descriptions required. The marks must be identified as "Not Occupiable"; and D. Log files to include within the submittal to NGS. ADDITIONAL SERVICES The SURVEYOR will provide, as requested, additional services that require analyses beyond those described in the above task(s). These additional services may include, but not limited to: A. Additional surveying services; B. Additional data formatting and submittal to agencies other than specifically stated herein; and C. Access to restricted areas, or remote areas requiring special access and/or security clearance. FEES AND BILLING This proposal, as well as the attached General Contract Provisions, represents the entire understanding between the COUNTY and the SURVEYOR concerning the PROJECT. Billing will be invoiced PER MONUMENT and on a monthly based on percentage complete for Lump Sum Tasks and for services actually provided for Time and Material (hourly) Tasks. Invoices are payable as per the original Professional Surveying and Mapping contractual agreement between the COUNTY and the SURVEYOR. All remittance should be sent to Creech Engineers, Inc., P.O.
