You are directed to make the following changes in the Contract Documents :
1 . Description : Add two Type " E " Inlets .
Reason : To adjust elliptical pipe route (along U . S . Highway No . 1 ) to avoid conflicts
with power poles and an existing water main .
The change in Contract Price is determined as follows :
ADDITION to the Contract Price = (2 Type " E " Inlets ) ($ 2 , 200 per Inlet)
= $4, 400 . 00
2 . Description : Additional labor to modify two Type " E " Inlets on site .
Reason : Inlets required modification due to adjustment of elliptical pipe route (along
U . S . Highway No . 1 ) to avoid conflicts with power poles and an existing water main .
The change in Contract Price is determined as follows :
ADDITION to the Contract Price = $ 990
3 . Description : Construct thrust blocks for an existing water main .
Reason : To protect an existing water main against resultant thrust from internal
pipeline pressure in order to allow the installation of a drainage structure near the
water main . The water main must be exposed in order to install the drainage
structure and the thrust blocks will prevent the water main ' s internal pressure from
blowing the fittings or pipe sections from the pipeline .
The change in Contract Price is determined as follows :
ADDITION to the Contract Price = $ 3 , 307 . 70
ADDITION to the Contract Time = 2 calendar days
4 . Description : Remove and replace an existing 15-inch corrugated metal pipe ( CMP )
at an existing drainage structure .
Reason : Since the construction drawings were prepared , a new pipe was added ( by
others ) to an existing drainage structure that the Contractor must connect to as part
of this project . This pipe must be removed to allow the Contractor to perform its
work and then the Contractor must reinstall the pipe .
The change in Contract Price is determined as follows :
ADDITION to the Contract Price = $ 621 . 50
