Amendment Number Five (45)
Dear Ms. Austin:
Enclosed, please find an Amendment #5 package for the above referenced grant. With this
amendment, the County is requesting a reinstatement of the grant and a three (3) month time
extension. This time extension request will extend the contract ending date from August 19,
2008 until November 19, 2008.
Currently, the Oslo Fire Station replacement project, Phase I of the Wabasso water project, and
Phase II of the Wabasso water project are completed; however, there have been delays beyond
the County's control associated with the Rockridge vacuum sewer system project. The County
experienced technical difficulties with the programming of the system and electrical issues.
These technical difficulties prevented the start up of the system. Until these were resolved, the
County had to discontinue residential hookups.
With the programming difficulties and electrical issues resolved, the County has moved forward
with the residential hookups. Crew sizes have been doubled by the contractor to complete the
residential hookups as quickly as possible. The County feels confident that the residential
hookups can be completed within the additional three (3) month extension.
As part of the Amendment 95 package, the County is submitting the following:
■ DCA Form — Modification 45 to Grant Agreement — three (3) copies (all with original
signatures)
■ DCA Form — Request for Amendment 07.02
■ Revised Attachment "B" — Activity Work Plans

MODIFICATION NUMBER 5 TO GRANT AGREEMENT BETWEEN THE DEPARTMENT OF
COMMUNITY AFFAIRS AND INDIAN RIVER COUNTY
(Note: Proposed section changes are highlighted in grey)
This Modification is made and entered into by and between the State of Florida, Department of
Community Affairs, (the Department'), and Indian River County, Florida, (the Recipient"), to modify
DCA Contract Number 06DB-3 C- 10-40-0 1 -W 14, award dated August 20, 2005, ("the Agreement').
WHEREAS, the Department and the Recipient entered into the Agreement, pursuant to which the
Department provided a grant of $ 7,500,000.00 to the Recipient under the Disaster Recovery Initiative
Community Development Block Grant ("CDBG") Program as set forth in the Agreement;
WHEREAS, the Department and the Recipient desire to modify the Agreement;
NOW, THEREFORE, in consideration of the mutual promises of the parties contained herein,
the parties agree as follows:
• Reinstate Agreement
1. The Agreement is hereby reinstated as though it had not expired.
• Extend Agreement
2, Paragraph 3, Period of Agreement is hereby revised to reflect an ending date of
November 19, 2008.
• Revise Activity Work Plan
3. The Attachment B, Activity Work Plan section of the Agreement is hereby deleted and is
replaced by the revised Attachment B, Activity Work Plan section, which is attached hereto and
incorporated herein by reference.
o Revise Program Budget and Scope of Work
4. The Attachment A, Program Budget and Scope of Work section of the Agreement is hereby
deleted and is replaced by the revised Attachment A, the Program Budget and Scope of Work,
which is attached hereto and incorporated herein by reference.

o Change in Participating Parties
5. The Attachment G, Special Conditions section, is hereby modified to delete all references
to " ," as the Participating Party, and replace them with
" as the Participating Party with the understanding that the Recipient
and the new Participating Party will enter into a Participating Party Agreement containing provisions and
caveats that meet or exceed the conditions agreed to in the Participating Party Agreement between the
Recipient and the original Participating Party.
o Inclusion of an Unmet Need as Addressed in the Original Application
a. The Attaclunent A, Program Budget and Scope of Work section of the Agreement is hereby
deleted and is replaced by the revised Attachment A, the Program Budget and Scope of Work,
which is attached hereto and incorporated herein by reference.
b. The Attachment B, Activity Work Plan section of the Agreement is hereby deleted and is replaced
by the revised Attachment B, Activity Work Plan section, which is attached hereto and
incorporated herein by reference.
o Change in Number of Accomplishments and/or Beneficiaries
6. The Attachment A, Program Budget and Scope of Work section of the Agreement is hereby
deleted and is replaced by the revised Attachment A, the Program Budget and Scope of Work,
which is attached hereto and incorporated herein by reference.
All provisions of the Agreement and any attachments thereto in conflict with this Modification
shall be and are hereby changed to conform to this Modification, effective as of the date of the execution
of this Modification by both parties.
All provisions not in conflict with this Modification remain in full force and effect, and are to be
performed at the level specified in the Agreement.
