All services performed under this contract shall be supervised and certified by a licensed
Professional Engineer (PE) registered with the State of Florida and staff employed directly with
your firm.
Scope of Services
All work shall be performed on a unit price basis in accordance with the attached Budget
Estimate dated June 30, 2008 and shall comply with Standards enumerated in the RENEWAL
OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT IRC PROJECT NO. 0534 WITH
DUNKELBERGER ENGINEERING & TESTING. INC. Effective Date: October 1, 2007.
Minimum Specification Requirements as follows:
Asphalt Coring Services
• Coring of the asphalt at Thirteen (13) locations as directed by Engineer of Record and
spaced at about 100 foot intervals within the subject roadways to determine asphalt
thickness.
• Preparation of a brief engineering data report documenting the findings of this work.
All work shall also comply with the current Florida Minimum. Technical Standards.
COMPENSATION.
The COUNTY agrees to pay, and the Geotechnical Professional agrees to accept, a fee for
services rendered according to the "Scope of Services" identified in the attached exhibit in a
Not -to -Exceed Amount of $1,505.00.

TIME FOR COMPLETION
This Work Order No. 6 shall be completed within 30 days from the execution of this Agreement.

As requested in your letter dated June 27, 2008,

referenced project.

for Asphalt Coring Services for the above
The Enclave Subdivision is located at 5910 65`x' Street in Vero Beach, Indian River County,
Florida. Reportedly, several areas of pavement deficiencies have been noted within the 1200
lineal feet of prciject roadway. As a result, Dunlcelberger has been requested to perform a limited
pavement evaluation to measure asphalt thickness at 100 foot intervals.
We will provide mobile coring equipment and a tvvo-man coring crew for one (1) day to perform
the field work,. Our scope of wort. Nvill uiclude the followiAg:
Pavement Cores
1. Coring of the asphalt at thirteen (1 3) locations as directed by Indian River County and
spaced at about 100 foot intervals within the subject roadways to measure asphalt
thickness.
2. Preparation of a brief engineering data report: documenting our findings.
Terms and Payment
We propose to bill this work on a unit rate basis in accordance with our current 2007-2008
Col?tllMing Geotechnical Services Co79tlYlct (IRC Project No. 0534). We understand this work
will be performed under a purchase order number issued by Indian River County,
Based on the described scope of work, we have provided an Estimated Scope and Baciget
(Attaclulnent A) for the requested services and established all estimated budget of $1,505.
