All services performed under this contract shall be supervised and certified by a licensed Professional Engineer ( PE ) registered with the State of Florida and staff employed directly with your firm . Scope of Services All work shall be performed on a unit price basis in accordance with the attached Scope of Services dated February 19 , 2009 and shall comply with Standards enumerated in the RENEWAL OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT IRC PROJECT NO . 0206 WITH DUNKELBERGER ENGINEERING & TESTING , INC . Effective Date : November 18 , 2008 . Minimum Specification Requirements as follows : Soil Compaction Concrete Compressive Strength Testing Asphaltic Roadway Surfacing Testing All work shall also comply with the current Florida Minimum Technical Standards . E . . . . via e- snail (mnixon c@iregov. corn) Subject : Construction Materials Testing Services 5th Street SW Bridge over the 43rd Avenue Canal IRC Project #0206 Indian River County, Florida Dear Mike : INTRODUCTION As requested , Dunkleberger Engineering & Testing, Ine . ( Dunkelberger) is pleased to submit this proposal for Construction Materials Testing Services during construction activities at 5th Street SW Bridge over the 43d Avenue project in Vero Beach (Indian River County), Florida . The project will involve re -construction and widening of the existing roadways at the intersection of 51h Street SW and 43r' Avenue, drainage and water main improvements within the project limits, and the construction of a new bridge along 5t1i Street SW over the 43 ' d Street Canal . SCOPE OF SERVICES The scope of work described in this proposal is based on our review of civil engineering drawings prepared by ARCADIS U . S . , Inc . , dated August 2008 , the structural bridge drawings prepared by Bridge Design Associates, Inc . , dated April 30 , 2007 , and our experience with similar projects for you in the past . We will provide the required construction materials testing services for the project the specific scope of our services will include the following : Fiel(IObseryation & Testinz Earthwork observations by a senior technician during the site grading operation including the proof-rolling of " strip grade" within the roadway alignment and embankment construction for the proposed bridge . • In-place density testing during placement of the roadway/embankment till . The density tests will be performed at a frequency of one ( 1 ) test for every 200 lineal feet of roadway/embankment within the proposed alignment, for each 1 -foot of backfill placed . PSL -07 - 2558 (BG 10 . 6 ) • In -place density testing during backfilling of storm drains and catch basins for each 1 ,& foot lift of backfill placed from the bottom of the structure up to an elevation corresponding to final grade . • In -place density testing during backfilling of the rigid and flexible pipelines . Below pavement areas , density tests will be performed at a frequency of one test location for each 200 lineal feet (or fraction thereof) of pipe . At each roadway or driveway crossing, density tests will be performed for each i400t lift of backfill placed starting at an elevation corresponding to the invert of the pipe and extending up to the final grade . • In-place density tests within the stabilized subgrade and base course layers at a frequency of 1 test for each 200 lineal feet of roadway . • In -place density tests at the subgrade for the approach slabs and sidewalks prior to concrete placement. • Sampling, slump testing, molding, transporting, curing, and laboratory testing to determine the compressive strength of concrete used for the bridge construction (pile caps , railings and approach slabs) as well as for the sidewalk and curbing. Laboratory Testing • Perform laboratory moisture- density relationship (Proctor) tests (AASHTO T- 180 and AASHTO T-99) and sieve analysis (percent fines) on the embankment fill and/or pipe backfill soils . • Limerock Bearing Ratio (LBR) tests for stabilized subgrade and base course within the roadways . Asphalt Paving & Testing • Asphalt plant observations and laboratory testing by a CTQP Plant Level I Asphalt Technician as the asphalt hot mix is produced at the plant . The laboratory testing will include extraction/gradation testing on selected asphalt samples . • Paving observations by a field technician to verify thickness , temperature , and mix design tickets during asphalt placement. • Coring of the asphalt at four (4 ) locations for subsequent laboratory testing to verify asphalt thickness and density . Engineering & Reporlin � • Engineering oversight during the construction to coordinate the aforementioned test program, make site visits , and attend progress meetings . ® ui lkelhergewV Indian River County Project # 0206XHIBI " Page 3 Project No . PSL- 07 - 2558 ( BG 10 . 6 ) 0 • Submittal of brief engineering reports documenting our findings and test results . The previous summarizes the typical testing services anticipated for the project . Dunkelberger can provide a complete range of construction services , including turbidity monitoring, vibration monitoring and specialty work , if needed . We understand that during the test pile program and the production pile installation the pile driving analyses and inspection will be performed by Bridge Design Associates, Inc . Our construction materials testing services will be provided on an as -needed basis as dictated by the pace and methods of construction . For scheduling purposes, we normally request at least 24 hours notice prior to the time the field services are needed . TERMS AND PAYMENT We propose to bill this work on a unit rate basis in accordance with our current 2008-2019 Continuing Geotechnical Seitvices Agreeinent. Based on the described scope of work and testing frequencies , we have provided an Estiniated Scope and Budget (Attachment A) for the required services and established a budget of $19, 994.
