FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Masteller, Moler, Reed & Taylor, Inc.
In accordance with Contract No. 0725 2007-2009
This Work Order No. 4 is in accordance with the existing AGREEMENT dated November 6`", 2007
between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County (COUNTY).
This Work Order No. 4 amends the agreement as follows:
SECTION I — PROJECT LIMITS
Begin at the terminus of the County Road 512 Phase IV project east of 1 -95 (currently under
construction), traveling west on County Road 512 through the intersection at North Broadway
Street in the City of Fellsmere. Length is approximately 3 miles.
(Reference attached project limits map)
This Work Order No. 4 is for the SURVEYOR to perform an updated Topographic survey for
engineering planning and design purposes of County Road 512 Phase V from the termination
point of County Road 512 Phase IV to approximately 1000 feet west of Broadway Street in the City
of Fellsmere, all lying in Indian River County, Florida. The original survey was performed under the
prior Work Order dated January 2005 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR)
and Indian River County (COUNTY).

SECTION 11 - SCOPE OF SERVICES
As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County, the
SURVEYOR shall provide Professional Land Surveying services to complete all tasks as outlined
in this Work Order No. 4 described as follows:
• Updated Route Survey:
1. LIMITS: Update existing route survey, 1-95 to Willow Street, last updated in 2005, AND extend
project approximately 1000' west of the Broadway Street Intersection.
2. Main CR512 Corridor: Perform - update topographic (route) survey of the CR512 phase V
corridor, route limits to include verification and/or survey of elevations, grade breaks (I. E. visible
high - low points), driveways, all visible utilities and drainage features and inverts (As-Builts)
within 200' feet and beyond existing right of way
• Buildings and/or building fronts and Finish Floors (low floor) along main corridor are to be
obtained within 100' feet of existing right-of-way.
3. Side Streets: Perform route survey of all side streets 500' feet beyond existing CR512 right of
way, and up to the existing building frontage or 50' feet beyond the existing side street right-of-
way, to include verification and/or survey of elevations, grade breaks, driveways, all visible
utilities and drainage features and inverts (As-Builts) within 50' feet and beyond existing right of
way.
• Buildings and/or building fronts and Finish Floors (low floor) along side streets are to be
obtained within 50' feet of existing right-of-way.
• Survey shall provide all Rights -of- Ways, jurisdictional lines and property and/or parcel lines as
they intersect or affect the corridor(s). Property lines shall be confirmed and mapped utilizing
sufficient land corner locations and/or deed interpretations.
4. Recover existing vertical and horizontal control and reestablish as necessary. Survey baseline
will be referenced to State Plane Coordinates, North American Datum 1983/90, Florida East
Zone. Elevations shall be referenced to the National Geodetic Vertical Datum of 1929.
5. Extend existing baseline of survey to 1000 feet west of Broadway Street, monument and set
reference points as required in original work order.
6. Recover existing monumentation along extended route and locate as necessary.
7. Perform cross sections at 100' foot intervals and/or change in grade (I.E. visible high -Low
points) along the extended route and side streets.
8. Provide Flood Zone lines and/or areas as per the current FEMA Flood Rate Maps.
9. This survey will not include any trees, vegetation or vegetation lines or areas of environmental
concern.
10. Update existing Base Plan sheets of existing survey and above described additional data
depicting all topography, features, baseline, R/W, cross sections and adjacent parcel
information.

SECTION III —TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
Survey shall be completed within one hundred twenty (120) business days from Notice to
Proceed.
2. The SURVEYOR shall provide the survey in the following format:
• Cover sheet with survey notes, certifications, location sketch and other pertinent data.
• Survey horizontal and vertical control sheets.
• Subsequent Topographic sheets.
3. The SURVEYOR shall provide the COUNTY with a preliminary (+/- 90%) survey within sixty
(60) days from the notice to proceed for review and comments. The COUNTY shall have 30
business days to review and comment.
4. The SURVEYOR shall provide the COUNTY three (3) executed copies (sets). ONE (set) in
standard black and white format, ONE (set) overlaid on the 2005 aerial photography currently
available, one Mylar of the survey, a digital file in AutoCAD and PDF of both standard and
aerial overly upon completion. The format of the survey, preparation of the Work Product and
digital versions shall be completed so that the COUNTY or other consultants can readily use it
for the design and analysis of the area, as defined. It shall contain all information necessary for
third -party surveyor to independently recreate and/or utilize the survey work. It is acknowledged
all final products become property of Indian River County and will be available for use by the
public at large.
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for the above described
services rendered as identified in Sections I, II, and III of this Work Order No. 4, a lump sum fee of
$443200.00.
All and/or any additional services not described hereon shall be pre -approved by the COUNTY.
Approved additional services shall be invoiced at the rates disclosed in the approved fee
schedule with the SURVEYOR for the contract year(s) of November 6'h, 2007 through
November 5'h, 2009.
All invoicing shall include project identification, Work Order No. 4, Contract No. 0725, itemize man-
hours and materials expended to complete the scope of services. Payments shall be in accordance
with the original Professional Surveying and Mapping/GIS Services Agreement, contract No: 0725
with the COUNTY and as stated in Section II, III and IV hereon.

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of the
AGREEMENT shall remain in full force and effect, and are incorporated herein.
IN WITNESS WHEREOF the parties hereto have executed these presents this 13th day of

Hereby requests and authorizes the following services:
Perform an updated Route survey for property located in Indian River County and identified as County Road 512
Phase V. The project limits are 500 feet east of 1-95 to 1000 feet west of Broadway Street in Fellsmere, Florida.
As agreed upon between Masteller, Molec Reed & Taylor, Inc. and Indian River County, the SURVEYOR. shall
provide Professional Land Surveying services to complete all tasks as outlined in this proposal described as follows:
SCOPE OF SERVICES:
• Update existing route survey, last updated in 2005.
• Main CR512 Corridor: Perform - update topographic (route) survey of the CR512 phase V corridor, route limits to
include verification and/or survey of elevations, grade breaks (I.E. visible high -Low points), driveways, all visible
utilities and drainage features and inverts (As-Builts) within 200' feet and beyond existing right of way
• Buildings and/or building fronts and Finish Floors (low floor) along main corridor are to be obtained with in 100'

• Survey shall be completed within one hundred twenty (120) business days from Notice to Proceed.
