AUTHORIZATION: This Feasibility Authorization Form ("Form") is issued by the Company to the Customer pursuant to that certain Master Agreement for Demand Side Management and Energy Efficiency Services ("Master Agreement"), effective as of 19th day of August, 2008. This Form authorizes the Company to commence Services as described herein pursuant to the terms and conditions of the Master Agreement. This Form is not intended as a Change and in no way amends, varies or modifies the Master Agreement. Any alternate, different or additional terms or conditions referenced by the Customer in subsequent correspondence from the Customer are hereby rejected and will not become part of this Form or other the Master Agreement unless expressly set forth and incorporated herein. In order for the Company to commence Service set forth in this Form, the Customer is required to sign this Form. Capitalized terms used herein without other definition shall have the meanings set forth in the Master Agreement. This Form consists of this Feasibility Study Authorization Form and the following attachments, which are incorporated into this Form by this reference: (i) Exhibit A—Feasibility Study, and (ii) Exhibit B—Feasibility Study Price. II. SCOPE OF WORK: The Company shall prepare a Feasibility Report which shall set for the Services reccommended ECO(s) for the implementation at the Service Location identifed above, as more particularly described in the Feasibility Study, attached hereto and made a part hereof as Exhibit A. III. FEASIBILITY STUDY PRICE: Subject to Section 3.5 of the Master Agreement, the Customer shall compensate the Company for the Services provide in connection with the creation and development of the Feasibility Report for the price set forth in Exhibit B. This Form is being provided by the Company to the Customer, acknowledging acceptance of the Services specified in this Form, and the Master Agreement, which is incorporated herein. The Customer has examined and carefully studied all of this Form, including the Master Agreement and all exhibits, appendices, specifications, terms and © 2006, FPL Energy Services, Inc., All Rights Reserved Attachment A Page l of 5 conditions thereto and hereby agrees to be bound by any and all terms, conditions, and obligations set forth therein. Office, 4055 41st Ave. Vero Beach, FL 32960 • Road & Bridge Office 4550 41 st Street, Vero Beach, FL 32960 A total of 234,000 square feet of Air Conditioned Space SCOPE OF SERVICES: Company will conduct a full financial grade Feasibility Study of the facilties listed above. The company will investigate Energy, Water and Maintenance savings opportunities and document all of the lighting, motion detectors and water conservation opportunties at each building site. LIGHTING Company will: • Identify and Evaluate all lighting use at each facility • Measure current light levels and compare to industry and County standards. • Recommend energy efficient replacement lighting • Determine the cost to install and the annual savings for the new lighting recommendations MOTION DECTECTORS Company will: • Investigate the opportunties to install Motion Dectectors to reduce the lighting energy usage • Determine the cost to install and annual the annual savings of the Motion Dectector installations WATER CONSERVATION Company will: • Identify and Evalaute all the water use devices at each facility • Recommend that all devices not meeting current water conservation criteria be upgraded to new or modified devices • Determine the cost to install and the annual savings of the Water Conservation devices SOLAR PHOTO VOLTAIC PANEL Company will: • Evalute the installation of a solar PV panel capable of providing power to light the courthouse parking garage during daylight hours and at the north library • Determine the installed cost to build the panel and connect the equipment to the exisiting lighting and electric service • Provide 30% level of the plans to construct the PV panel. • Determine the annual electric savings for the county by utilizing PV Panels © 2006, FPL Energy Services, Inc., All Rights Reserved Attachement A Page 3 of 5 SOLAR THERMAL PANELS Company will: • Evaluate the installation of a solar thermal panel capable of providing hot water to the Sheriff's Complex during daylight hours • Determine the installed cost to build the panel and connect the equipment to the existing water systems • Provide 30% level of the plans to construct the solar thermal panel. • Determine the annual electric savings for the county by utilizing the solar thermal panel The reports will also include financing options, savings guarantee, product specifications, and the calculations used to determine the savings. The company will also provide the total cost for the county for the company to implement these ECOs. HEALTH DEPARTMENT • Health Department 1900 27th Street, Vero Beach, FL 32960 • FPLES will initiate monitoring of this building during the Phase 1 period. The results will be utilized in Phase 2. CUSTOMER WILL PROVIDE: • Utility bills for a minimum of the 24 previous months • Access to buildings • Copy of building plans if necessary.
