HomeMy WebLinkAbout2009-271 0 % 3 /oq ISO 3 AMENDMENT NUMBER 1 TO WORK ORDER NUMBER 15 This Amendment 1 to Work Order Number 15 is entered into as of this 13th day of October 2009 ( " Effective Date " ) pursuant to that certain Continuing Contract Agreement for Professional Services entered into as of 10 August , 2004 , and amended effective 1 August 2009 ( as so amended , the "Agreement" ) , by and between Indian River County Solid Waste Disposal District , a dependent special district of Indian River County , Florida , ( " District" ) and Geosyntec , Inc . ( " Consultant" ) . 1 . The District has selected the Consultant to perform the professional services set forth in existing Work Order Number 15 , Effective Date 15 August 2008 , 2 . The District and the Consultant desire to amend this Work Order as set forth on Exhibit 1 attached to this Amendment and made part hereof by this reference , The professional services will be performed by the Consultant for the fee schedule set forth in Exhibit 1 and within the timeframe more particularly set forth in Exhibit 1 , all accordance with the terms and provisions set forth in the Agreement . 3 . From and after the Effective Date of this Amendment , the above - referenced Work Order is amended as set forth in this Amendment . Pursuant to paragraph 1 . 4 of the Agreement , nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth herein . Mehta : In response to your request for a current budget evaluation regarding the Indian River County ( IRC) Aboveground Storage Tank ( AST) project ( Work Order No . 15 ) , Geosyntec Consultants , Inc . (Geosyntec) has prepared this letter and cost table ( Table 1 ) in Attachment 1 . Attachment 1 includes an upAo - date listing of items not included in the original Work Order No . 15 and assigns their individual costs within the requested change order budget . For clarification , an itemized breakdown of the original proposal ' s Construction Contingency budget of $ 15 , 760 approved by IRC is shown in Attachment 1 . Items associated with the requested change order budget include the following : • Additional design , administration , and correspondence preparation effort required by Geosyntec for the permitting process and project completion (see Items 2 , 4 , 5 , 15 , and 17 on attached Attachment 1 ) . • Additional subcontractor costs associated with work/equipment not included in the original scope and Geosyntec ' s effort regarding coordination , oversight , etc , associated with the additional subcontractor scope costs ( see Items 6 through 14 , 16 , and 18 on attached Attachment I ) . 130CA/FRI545 AST . Page 2 Please note that Item 19 on Attachment 1 reflects a credit for IRC to the overall project budget, which relates to the Gifford Road AST Sampling/ Relocation (Task 2) as indicated in Exhibit 2 of IRC ' s Work Order No . 15 . Based on recent communications with Charles Vogt , IRC Department of Health , the closure inspection for the MOT site is complete and we will not have to complete the closure sampling as originally scoped . However, some type of reporting document that will include observations and tank tightness testing documentation will still be required for this Task . Therefore , we have estimated approximately $2 , 100 to be representative of the effort and expenses associated with the sampling activities that are no longer required for the project , which has been incorporated as a credit to IRC in the attached table . SCHEDULE To date , work associated with Phases 1 , 2 , 6 and 7 of Work Order No . 15 have been completed , except for limited reporting documentation preparation associated with Phase 2 (Gifford Road Sampling Relocation ) . The limited reporting required for Phase 2 is expected to be prepared within the same timeframe as the reporting required in Phase 5 ( IRC Landfill Closure Report ) . Phase 3 ( IRC Landfill AST Construction and Installation) and Construction Contingency activities included in Work Order No . 15 are mostly completed and AST startup activities are expected to be initiated in early October 2009 . Once the AST relocated to the landfill is operational , activities will be initiated for the remaining Work Order No . 15 scope items , which include Phase 4 ( IRC Landfill Existing AST Closure) and Phase 5 ( IRC Landfill AST Closure Report) . Please note that items in Change Order Request No . 1 include tasks that have been completed to - date or tasks that are to be completed in conjunction with Phase 3 ( IRC Landfill AST Construction and Installation ) . 130CA/ FRI 545 ASf .
