This Work Order Number 21 is entered into as of this5thday of May 2009 pursuant
to that certain Continuing Contract Agreement for Professional Services entered into as of
August 10 , 2004 and amended July 24 , 2007 ( as so amended , the "Agreement" ) , by and
between Indian River County Board of County Commissioners ( " COUNTY" ) on the same terms
as the existing Agreement , as extended , between the Indian River County Solid Waste Disposal
District , a dependent special district of Indian River County , Florida , ( " DISTRICT' ) and
Geosyntec , Inc . , a Florida corporation ( " CONSULTANT' ) .
The COUNTY has selected the Consultant to perform the professional services set forth
on Attachment A , attached to this Work Order and made part hereof by this reference . The
professional services will be performed by the Consultant for the fee schedule set forth in
Attachment B , attached to this Work Order and made a part hereof by this reference . The
Consultant will perform the professional services within the Schedule more particularly set forth
in Attachment A , attached to this Work Order and made a part hereof by this reference all
in
accordance with the terms and provisions set forth in the Agreement . Pursuant to paragraph
1 . 4 of the Agreement , nothing contained in any Work Order shall conflict with the terms of the
Agreement and the terms of the Agreement shall be deemed to be incorporated in each
individual Work Order as if fully set forth herein .

Dear Mr. Williams :
Geosyntec Consultants ( Geosyntec ) is pleased to submit this fee proposal to Indian River County
( IRC ) for professional services associated with the closure of two , 10 , 000 gallon steel
underground storage tanks ( USTs ) and the electrical connections for two , IRGsupplicd, 500-
gallon Convault aboveground storage tanks ( AST) equipped with dispenser pumps and delivered
with pre- cast slabs at the IRC Sheriff' s Department Headquarters . It is Geosyntec ' s
understanding that the 10 , 000 gallon USTs has been used for the storage of unleaded gasoline ,
exclusively. The scope of work described below has been developed in accordance with Florida
Department of Environmental Protection ( FDEP ) Storage Tank System Closure Assessment
Requirements and Chapter 62 - 761 , Florida Administrative Code ( FAC ) . An overview of the
work completed to date and the proposed project scope follows .
OVERVIEW
Geosyntec performed groundwater sampling in the four monitoring wells surrounding the
10 , 000 - gallon USTs in June 2007 . The groundwater sampling was performed to assess whether
the USTs have potentially had a detectable release of petroleum compounds to groundwater. The
laboratory analytical results , presented in a letter report dated 23 July 2007 , did not detect any
petroleum constituents above groundwater cleanup target levels ( GCTLs) . Based upon these
results , it does not appear that the USTs are compromised or that a release of petroleum
compounds has occurred . Accordingly, the scope of work developed for the site assumes that the

treatment of petroleum - contaminated groundwater from within the UST excavation will not be
performed and that the over- excavation of the UST area to remove petroleum - impacted soils will
not be required .
The proposed scope of work for the services required to close the USTs and provide electrical
connections to the IRC -provided ASTs has been broken down into three separate phases ( Phases
A through C ) as described below .
Phase A — Preliminary Activities
Phase A includes project management for the entire project ( Phases A through C ) , as well as all
preliminary activities associated with meeting with and securing project costs from three licensed
petroleum storage systems contractors and modifying the Site- Specific Health and Safety Plan to
cover the UST closure activities . Specific activities include :
• project planning and management responsibilities , such as correspondence with IRC ,
IRC Department of Health , subcontracting, invoice review , project coordination, and
project administration ; and
• participating in on- site meetings and correspondence with IRC , IRC Department of
Health , and Brevard County Natural Resources Management Office ( administers
petroleum cleanup progpin in IRC ) .
Phase B — UST Closure and AST Connection Field Activities
Phase B includes project activities associated with implementing the UST closure activities and
connecting the IRC - provided ASTs , including the following tasks :
• Perform oversight and monitoring of activities associated with the proper closure and
off- site disposal of the two , 14, 000- gallon USTs in accordance with Chapter 62 - 761 ,
FAC . Tasks to be performed by Geosyntec ' s subcontractor will include the following :
o secure permits for the removal activities ;
o sawcut and remove concrete over the UST area ;
o excavate overburden and stockpile for fill material ;
o remove any liquid residue/ sludge within USTs ;
o properly remove and dispose of USTs at the IRC landfill ;

o cut and cap product and vent line piping at the edge of the excavation
( i . e ,
adjacent to the existing canopy which will not be removed ) ;
o backfill area with existing fill and balance of material required to bring
excavation to grade ; and
o supply, install and broom finish an area up to 35 ft by 40 ft by 6 - inches thick with
a 3 , 000 psi fibermesh concrete .
• Perform organic vapor analyzer screening of excavated soils during the performance of
the UST closure activities .
• Upon removal of the USTs perform soil sampling ( up to four excavation side-wall
samples ) at the location of the 10 , 000 - gallon USTs . Collected soil samples will be
laboratory analyzed for benzene , toluene, ethylbenzene, total xylenes , and methyl tert -
butyl ether ( BTEX/ MTBE ) , polynuclear aromatic hydrocarbons ( PAHs ) , and total
recoverable petroleum hydrocarbons (TRPH ) ( one location) .
• Install a temporary well screen in the excavation and collect groundwater samples from
the center of the excavation area for BTEX/ MTBE , PAHs , and TRPH .
• Provide electrical connection to two , IRC -provided , 500-gallon ASTs that will be
delivered and placed on pre- cast concrete slabs ( provided by manufacturer) .
Geosyntec obtained estimates from three licensed petroleum storage systems contractors
( Attachment 2 ) , based upon the specific Schedule of Prices table supplied by Geosyntec for
project bidding . Geosyntec received three subcontractor bids, with base bid costs of $ 26 , 125 ,
$ 29 , 970 and $ 36 , 800 . Based upon the estimates submitted , Geosyntec will subcontract the low
bidder, Petroleum Resources & Development ( PR & D ) to perform the UST closure services and
will perform oversight of the subcontractor during field activities , conduct required soil and
groundwater sampling activities and perforin organic vapor analyzer ( OVA) screening of the
excavated soil .
Due to the uncertainties associated with the UST closure projects and unknowns associated with
the 500- gallon ASTs , a 20 percent contingency has been added to Task element B . The
contingency is in place to cover items such as increased excavation volumes , costs for vacuum
truck on site time , increases in petroleum - contact water disposal volumes , increases in project
duration due to weather, increased excavation volumes , additional concrete beyond the 1 , 400

square feet proposed , etc . Geosyntec will notify IRC and secure approval of any field conditions
that may require access to contingency fees . Geosyntec ' s proposal assumes up to six days of
oversight for field activities associated with the UST closure and AST connection activities . A
breakdown of estimated project costs is included as Attachment 3 .
Phase C — Reporting
GeoSyntec will prepare a Tank Closure Report in accordance with Chapter 62 - 761 , FAC and
FDEP Storage Tank System Closure Requirements . The report will include photographs
documenting the tank closure work , laboratory analytical results for collected soil and
groundwater samples in tables and figures , and a detailed summary of the field activities
completed at the site . The final reports ( four hard copies and one electronic copy to be provided
to IRC ) will be signed and sealed by the Florida - licensed professional engineer in responsible
charge of the project . A breakdown of estimated project costs is included as Attachment 3 .
ASSUMPTIONS
The following assumptions were made in developing this fee proposal :
• IRC personnel will assist Geosyntec with the identification of and protection (if
necessary) of any utilities located within the UST excavation area .
• Geosyntec will not be responsible for damage to any underground utilities located on the
site ( it is presumed that any utilities encountered during UST closure activities will be
cut/ capped and/or removed for off-site disposal ) .
• No costs are provided for the removal of the overhead canopy.
• The USTs will be drained of fuel ( to the greatest extent feasible) prior to UST closure
field activities .
• The USTs , product piping, concrete , and miscellaneous debris will be transported to the
IRC Class I landfill for disposal as non - hazardous waste . Landfill disposal charges will
be handled directly by IRC .
• All site work can be performed in Level D personal protective equipment .

• The 500- gallon ASTs will be provided by IRC and will be delivered/off- loaded by IRC
and /or the AST manufacturer and placed on pre-cast manufacturer provided slabs and/or
on a suitable concrete surface .
• The 500- gallon ASTs will come equipped with product dispensers and will only require
electrical connections .
• Geosyntec will be able to work unimpeded .
SCHEDULE
Geosyntec anticipates that upon approval from IRC , UST closure field activities will be initiated
within six weeks . It is anticipated that the UST closure activities will require approximately one
week . The AST connection field activities will be coordinated with IRC based upon the
procurement and delivery of the ASTs to the site. The final Tank Closure/Source Removal
Reports will be provided to IRC within six weeks of completion of field activities .
ESTIMATED BUDGET
The fee proposal is provided as Attachment 3 . The total estimated budget to complete to work
described in the fee proposal is $60,067 .

UCC is pleased to provide you with bid quote for the removal of two 10,000 gallon UST as per
your specifications provided for the above referenced site on your enclosed bid farm with the
following conditions:
1 . Bid assumes there will be no contaminated soil frosn the excavation.
Contaminated soil will be Dandled as additional work at an additional expense.
2. No dewatering is included in this bid. If water is encountered then the excavation
will need to be backfilled with gravel to the water table. Geotextile fabric laid on top
of the gravel and then backfilled with clean soil . Gravel will be provided for an
additional $35 .00 per ton plus $ . 50 square foot for fabric .
3 . Existing canopy will remain in place.
4 . VAC truck services will be required for this job . The additional costs are $ 120. 00
per hour, portal to portal and rinsates and liquids disposed at $ . 65 per gallon. Estimated
time for VAC truck services is 8 hours and liquids and rinsates are estimated at 600 -
800 gallons .
S . Bid assumes that Indian River County will waive permit fees and that the TRC
land fill will waive all tipping fees for concrete debris, construction debris and USTs.
