In accordance with Contract No . 0725 2007 -2009 This Work Order No . 6 is in accordance with the existing AGREEMENT dated November 6th 2007 between Kimley- Horn and Associates , Inc . ( SURVEYOR) and Indian River County ( COUNTY) . This Work Order No . 6 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 6 is for the SURVEYOR to perform all related field and office Surveying and Mapping services in connection with the Indian River Boulevard Sidewalk Improvements Project , from 12th Street to 17th Street . Detailed information of the existing 14th Street Canal Bridge is required for design and permitting of the project . SECTION II - SCOPE OF SERVICES As agreed upon between SURVEYOR and the COUNTY , the SURVEYOR shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 6 , and per the attached Scope of Services and cost outline ( Exhibit A) . Bridge Detail Survey March 24 'h2009 Agenda Page 2 of 3 SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT 1 . Project shall be completed as follows : 2 . Time of FINAL project completion shall be within 14 calendar days of Notice to Proceed . 3 . Deliverables -The SURVEYOR shall provide the COUNTY : a . Five ( 5 ) paper " hardcopy ' s " 8 '/2 " x 11 " sheet "West Side Elevation Exhibit" and related digital AutoCAD and PDF files . b . Work Product and digital versions are to be prepared and submitted so that the COUNTY or other consultants can readily use it for the design and analysis of the area , as defined . It shall contain all information necessary for third - party surveyor to independently recreate and/or utilize the survey work . It is acknowledged all final products become property of the COUNTY and will be available for use by the public at large . c . The SURVEYOR ' S work product shall meet or exceed the minimum standards as defined by Sections II , III and IV . SECTION IV — COMPENSATION The COUNTY agrees to pay , and the SURVEYOR agrees to accept a total lump sum fee of $4 , 380 . 00 for the above described services rendered as identified in Sections I , II , and III of this Work Order No . 6 . All and /or any additional services not described hereon shall be pre - approved by the COUNTY . Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the contract year( s ) of November 6 , 2007 through November 5 , 2009 . All invoicing shall include project number ( 0745) , Work Order No . (W06 ) and Contract Number ( 0725 ) . Payments shall be in accordance with the original Professional Surveying and Mapping /GIS Services Agreement , contract No : 0725 with the COUNTY and as stated in Section II , III and IV hereon . ("KHA") is pleased to submit this letter agreement (the "Agreement") to Indian River County ("the Client") for providing survey and mapping services . Our scope of services, schedule, and fee are below . Scope of Services Task I — Preparation of an exhibit of the west side of the existing bridge over 14`h Street canal This task will consist of the following: • Research tidal information from the FDEP database. • Field location and sketches of the west side of the bridge including the beam, bents, end bents, piers and rip-rap . • Obtain elevations on the beam, bents, end bents, piers and rip-rap . Obtain elevation cross section of the canal at the west face of the bridge . • Reduce field data and prepare a " West Side Bridge Elevation Exhibit" on an 8 %2 " x 11 " sheet . • Calculate horizontal and vertical clearances between the piers and beam at mean high water and mean low water elevations . • Obtain sufficient elevations and dimensions to respond to questions 4 , 5 , 9b and 12 on the Department of Homeland Security, U . S . Coast Guard " Bridge Project Questionnaire " . • One revision will be made to the exhibit based upon review comments . Deliverables Deliverables will consist of 5 copies of the "West Side Bridge Elevation Exhibit". Schedule We will provide the "West Side Bridge Elevation Exhibit" to the County within 10 business days from written notice to proceed . KHA will perform the Scope of Services for a lump sum fee of $4, 380 . All permitting, application, and similar project fees will be paid directly by the Client. Closure In addition to the matters set forth herein, our Agreement shall include and be subject to, and only to, the terms and conditions in the existing AGREEMENT dated November 6, 2007 , between Kimley-Horn and Associates, Inc . , (SURVEYOR) and Indian River County (COUNTY) under the contract number 0725 .
