Civil Engineering and Land Surveying for Intersection
Improvements at 58th Avenue and State Road 60
AMENDMENT NO . 4 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING
SERVICES AGREEMENT BETWEEN KIMLEY - HORN AND ASSOCIATES , INC . AND
INDIAN RIVER COUNTY , FLORIDA.
This is an amendment to the existing Engineering/Land Surveying Services Agreement
(AGREEMENT) dated February 22nd , 2005 between Kimley- Horn and Associates , Inc .
(ENGINEER) and Indian River County (COUNTY) . This amendment addresses changes in
" Section I — Project Limits and Description" , " Section III - Scope of Services " and " Section V -
Compensation " of the AGREEMENT .
Amendment Description
This Amendment includes the following :
" SECTION I - PROJECT LIMITS AND DESCRIPTION " is being modified to incorporate the
following :
Consultant services are required for the preparation of an interim roadway improvement
plan for the 58 `h Avenue corridor . These services consist of supplemental survey and
interim roadway improvement plans at the 58th Avenue/ State Road 60 intersection in
unincorporated Indian River County , Florida . The interim roadway improvements
identified by the COUNTY consist of the following :
• Closure of the existing full access median opening located along 58t" Avenue
between State Road 60 and College Lane .
• Increase the vehicular storage capacity associated with the northbound dual
left turn lanes at the State Road 60/ 58t Avenue intersection .
• Design a southbound left turn directional median opening along 58th Avenue
between State Road 60 and College Lane . This directional median opening
shall be designed to accommodate a u-turn vehicular movement .
" SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following :
B . Supplemental Desmon Survey
The ENGINEER shall collect supplemental survey data along 58t" Avenue to
depict the current access condition associated with the newly constructed
commercial plaza located along the east side of 58t" Avenue between State Road
60 and College Lane . This task will consist of supplementing the digital base

Technical Standards set forth by the Florida Board of Professional Land
Surveyors in Chapter 61G17 -6 , Florida Administrative Code , pursuant to Section
472 . 027 , Florida Statutes .
C . Roadway. Plans
Roadway plans shall be prepared to include : plotting of survey data ; establishment of
profile grades ; key map ; plan and profile sheets (including geometric calculations ) ;
typical section sheets ; summary of quantities ; cross -section sheets at 100 ' intervals
( including earthwork computations ) ; signing and pavement marking plans ; and other
detail sheets necessary to convey the intent of the scope of services outlined herein . All
plans shall be prepared in the English units .
The Roadway plans will be submitted to the COUNTY for review at the 60 % and 100 %
plan completion .
" SECTION V — COMPENSATION " is being modified to incorporate the following :
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered
pursuant to this Agreement fees inclusive of expenses in accordance with the following :
A . Professional Services Fee
The basic compensation mutually agreed upon by the ENGINEER and the
COUNTY is as follows :
Task Labor Fee
Lump Sum Components
Supplemental Topographic Survey $ 1 , 500
Roadway Design $ 28 , 830
" SECTION XVI — DURATION OF AGREEMENT " is being modified to incorporate the
following :
The time for completion of the above described scope of services detailed in this
amendment will be one ( 1 ) month following the notice to proceed from the COUNTY .

The AGREEMENT is hereby amended as specifically set forth here in . All other sections of the
AGREEMENT shall remain in full force and effect and are incorporated herein .
This Amendment No . 4 to the AGREEMENT regardless of where executed , shall be governed
by and construed by the laws of the State of Florida .
In witness whereof the parties have executed this Amendment this 1st day of
December 2009 .
