In accordance with Contract No, 0725 3-II-a� 7 D. This Work Order No. 5 is in accordance with the existing AGREEMENT dated November 6, 2007, between Kimley-Horn and Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This Work Order No. 5 amends the agreement as follows: SECTION I — PROJECT LIMITS IRC Project 0749 SR 60 & 58th Avenue limits. Proposed Intersection limits (Reference attached location map): North - 26`h Street, South — Main Relief Canal, West— +/- 1,200', East - 55th Avenue SECTION II — SCOPE OF SERVICES The scope of services for this Work Order No. 5 is for the purpose of providing Right -of -Way staking and Boundary Survey professional services to support Right -of -Way Acquisitions, Planning, Design, Construction and permitting applications. Services shall include (not limited to): • Staking of existing and proposed Right -of -Way • Boundary Surveys • Sketch and Descriptions • Field an/or office positioning of easements and utilities. • Miscellaneous services as requested by County Staff. Boundary Surveys, staking, marking of parcels rendered under this work order are to be ordered and completed on an "as needed" basis. Kimley-Horn SR60 & W Ave Mise ROW Services March V, 2008 Page 2 of 4 SECTION III —TIME FOR COMPLETION/DELIVERABLES As defined in each individual service request. a) The SURVEYOR'S work product shall meet or exceed all scope of services and the minimum standards as defined by Chapter 472, Florida Statutes and Chapter 61G17, Florida Administrative Code and made a part of this agreement, or the COUNTY will not approve the SURVEYOR'S request for payment. SECTION IV — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered according to the "Scope of Services" identified in Sections I, II and III of this work order on a Time and Material basis and/or per parcel basis as defined in the individual request. Time and Material shall be a per the fee schedule for the Annual Surveying and Mapping GIS contract No. 0725 for the years 2007-2009. Combined services shall not exceed $15,000.00. All and/or any additional services not described hereon shall be pre -approved by the COUNTY. Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the contract year(s) of November 6th, 2007 through November 5th, 2009. All invoicing and payments shall be in accordance with the original Professional Surveying Services Agreement for Annual Surveying and Mapping Services, Contract No. 0725 with the COUNTY and as stated in Sections I, II, III and IV hereon. All invoicing shall include project identification, work order (WO 5) and contract number (0725), services performed, itemize per parcel and/or man-hours and material expended to complete the scope of services. Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services Agreement, contract No: 0725 with the COUNTY and as stated in Section Il, III and IV hereon.
