In accordance with Contract No . 0390 This Work Order No . 6 is in accordance with the existing AGREEMENT dated September 23 , 2003 , and the extension thereof, between Kimley-Horn & Associates , Inc . ( SURVEYOR) and Indian River County (COUNTY) . This Work Order No . 6 amends the agreement as follows : SECTION I — PROJECT LIMITS Kings Highway Extension South (58`h Avenue) , Phase I — the SURVEY LIMITS shall begin at a point on Kings Highway that is 150 ' north of Oslo Road and 9t" Street S . W. and continue south on Kings Highway to a point that is 1500 ' south of 17th Street SW. Oslo Road will be surveyed from a point 150 ' west of Kings Highway to the east end of the bridge over Lateral ` B " Canal . 17th Street SW will be surveyed 500 ' west of Kings Highway . Centerline profile elevations of the 17 'h Street SW roadbed lying east of Lateral ` B " Canal will be acquired for a distance of 1200 ' . 13th Street SW will be surveyed 500 ' east and 500 ' west of Kings Highway . SECTION II — SCOPE OF SERVICES The survey will include the establishment of a survey baseline with horizontal control set every 500 ' and vertical control set every 1000 ' (on the reference points outside the proposed area of construction) , location of above ground improvements and trees 6" diameter and greater at breast height, lying within the right-of-way of Kings Highway, 13th Street SW and 17th Street SW . Surface evidence of underground utilities will be located. Cross-section elevations will be obtained at 100 ' intervals and visible high and low areas along Kings Highway from the westerly toe of slope of Lateral ` B " Canal to a point 75 ' west of the right of way line . The delineation of tree lines and vegetation of concern will be shown within the additional 75 ' strip . Cross- section elevations of the entire canal right-of-way will be obtained at 500 foot intervals . Centerline profile elevations of the 17th Street SW roadbed lying east of Lateral ` B " Canal will be acquired for a distance of 1200 ' . Centerline profile elevations along the driveways and the lowest floor elevations for those homes lying west of Kings Highway are included . Drainage structures will be located and rim/invert elevations obtained. If a structure is inaccessible or the drainage pipes are recessed, an elevation on the bottom of the structure will be determined. Deliverables will include digital and signed/sealed copies of the Topographic Survey and Horizontal and Vertical Control Survey . Final submittal will be delivered to the County within 30 calendar days of receipt of the 80 % submittal comments . SECTION IV - COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered according to the " Scope of Services " identified in Section III of this Work Order No . 6 , a Lump Sum of $24 , 910 . 00 . The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein .
