HMM MASTELLER, MOLER & REED, INC. <br /> - SURVEYORS & MAPPERS <br /> April 29, 2004 <br /> Mr. Michael O 'Brien, PSMEXHIBIT , ` All <br /> Indian River County, Surveyor <br /> 1840 25th Street <br /> Vero Beach, FL 32960 <br /> RE : Response to request for proposal / Proposed 39th Avenue corridor between 69th & <br /> 71st Street. <br /> Dear Mr. O ' Brien: <br /> We thank you for the opportunity to offer the expertise and experience of Masteller, Moler & <br /> Reed, Inc . relative to the proposed referenced project. Upon field investigation of the site, <br /> Masteller, Moler & Reed, Inc. proposes to furnish the following: <br /> Perform a route survey for the purpose of design of a roadway. The project area is as <br /> follows : <br /> 1 ) 600 feet East & West of the proposed centerline of the 39th Avenue corridor along 69th <br /> Street. <br /> 2) 600 feet West of the proposed centerline of the 39th Avenue corridor and east to the East <br /> right-of--way line of the Old Dixie highway along 71st Street. <br /> 3 ) The proposed 39th Avenue corridor (200 ' wide) located between 69th and 71s' Street per <br /> documentation as provided by the County. <br /> Generally the limits of the survey within the project area are as follows . <br /> 4) 50 feet North & South of the right-of-way lines of 71 " Street along the project area as <br /> described in paragraph #2 above . <br /> 5) 50 feet North & South of the right-of-way lines of 69th Street along the project area as <br /> described in paragraph # 1 above . <br /> 6) 100 feet each side of the proposed centerline of the 39th Avenue corridor. <br /> The scope of services to be provided for this project will be as follows : <br /> A) Establish horizontal baseline control (or reference to) tied to sectional and/or established <br /> Boundary/Parcel lines. Control shall be set as to allow for reestablishment for <br /> construction purposes in the future. <br /> B) Establish in the field, control point ' s minimum 500 ' intervals . (State Plane NAD83190) <br /> C) Establish vertical control at minimum 500 ' intervals .
