NEC NEC Unified Solutions, Inc. <br /> Appendix A - Service Responsibilities <br /> Customer and NEC agree to the following specific responsibilities required for this project. <br /> Phase 1 : Acceptance of SOW <br /> 1 . Acceptance of SOW <br /> NEC shall be responsible for the following : <br /> 1 . 1 . Provide and sign contract documents . <br /> 1 . 2 . Upon Customer' s acceptance of SOW , invoice services based on progress of project <br /> and upon each project acceptance . <br /> Customer shall be responsible for the following : <br /> 1 . 3 . Review and sign contract documents . <br /> 1 . 4 . Submit services payments based upon progress plan <br /> Phase 2 : Site Readiness / Equipment Delivery <br /> 2 . Site Readiness <br /> NEC shall be responsible for the following : <br /> 2 . 1 . Provide a Project Manager to serve as NEC ' s contact in all aspects of this <br /> engagement , including scheduling , defining requirements , change control , escalation , <br /> implementation planning and acceptance . <br /> 2 . 2 . Conduct site survey to determine network and site readiness prior to implementation <br /> and deliver recommendations to Customer. <br /> Customer shall be responsible for the following : <br /> 2 . 3 . Provide a single contact for all aspects of this engagement, including scheduling , <br /> defining requirements , ensuring delivery of necessary information and access , <br /> ensure site readiness , change control , escalation , storage areas , implementation <br /> planning and acceptance . Any schedule changes must be made at least ten ( 10 ) <br /> business days prior to any scheduled activity . <br /> 2 . 4 . Make changes as recommended by site survey prior to implementation . <br /> 2 . 5 . Provide adequate environmental conditions as per manufacturer' s specifications for <br /> all equipment to be installed . <br /> 2 . 6 . Provide proper access to the site for Product installation . <br /> 2 . 7 . Order, install , and test all data and voice circuits prior to scheduled installation date . <br /> Ensure that Telco demarcations and circuits are clearly identified ( if not supplied or <br /> managed by NEC ) <br /> 2 . 8 . Identify locations for Product , rack/cabinets , circuits , and any data connections that <br /> will be made to the system . _ <br /> 2 . 9 . Provide a Letter of Agency and contact information for each vendor where NEC will <br /> act as the primary interface on behalf of the Customer ( as applicable ) . <br /> 2 . 10 . Provide a PBX , Telco , network , and/or cabling technician to be available during <br /> installation and testing ( if not supplied or managed by NEC ) . <br /> 2 . 11 . Install and verify the operation of all Customer Provided Equipment ( CPE ) not <br /> provided by NEC . Provide system documentation for CPE , if requested . <br /> 2 . 12 .
