Exhibit A <br /> AE Contract Amendment Number Four to the <br /> Standard Form of Agreement between Owner and Architect <br /> AIA Document B141 - 1997 and General Conditions <br /> of the Contract for Construction, 1997 edition of <br /> AIA Document A201 as modified. <br /> Between <br /> Owner: Indian River County Board of County Commissioners <br /> and <br /> Architect : Donadio & Associates, Architects of Vero Beach, Florida <br /> Executed May 13, 2003 <br /> For the following Project : <br /> Indian River County New Administration Complex to be generally located on approximately 13 acres north <br /> of 26" Street east of 20th Avenue south of Aviation Boulevard and west of Lateral "E" Canal in Vero Beach. <br /> Florida. This Project shall also include but not be limited to other such improvements as the Extension of <br /> 20th Avenue to the North and construction of a Bridge across the Main Relief Canal connecting 20th Avenue <br /> with Aviation Boulevard. <br /> THIS CONTRACT SHALL HEREINAFTER BE KNOWN AS THE "PRIMARY CONTRACT" <br /> OTHER AMENDMENTS <br /> AMENDMENT NO. 1 : <br /> On June 22 , 2004, the Board of County Commissioners approved Amendment No. 1 for the design of a third <br /> building to be located on 43rd Avenue which will accommodate the Supervisor of Elections main office, <br /> Buildings and Grounds and Animal Control facilities <br /> AMENDMENT NO. 2 : <br /> On Tuesday September 13 , 2005 , the Board of County Commissioners approved Amendment No. 2 for the <br /> design of a fourth building, Emergency Operations Center (second facility to be located on 43`d Avenue) to <br /> be located on 43`d Avenue which will accommodate the Emergency Services department. <br /> AMENDMENT NO. 3 : <br /> On July 11 , 2006, the Board of County Commissioners approved Amendment No. 3 for the design of certain <br /> modifications to the Primary Contract including the deletion of the Emergency Operations Center function <br /> from Building 2 and related improvements to convert the former EOC space into shell space for future <br /> expansion. Also included was design of a 10,000 square foot second floor area on a portion of Building 2 <br /> which was formerly one story. <br /> THE REQUESTED AMENDMENT ( AMENDMENT NO. 4) MODIFIES THE SCOPE OF WORK <br /> AS FOLLOWS : <br />
