On Tuesday November 11 , 2005 , the Board of County Commissioners approved a request to proceed with <br /> design and modification the meeting room in the former Supervisor of Elections space in Building 2 with the <br /> understanding that staff will return for Board approval of an appropriate Change Order as needed. This <br /> particular request is for an Amendment to the agreement with Donadio & Associates for the necessary design <br /> to accomplish this work . <br /> THE REQUESTED AMENDMENT ( AMENDMENT NO. 4) MODIFIES THE SCOPE OF WORK <br /> AS FOLLOWS : <br /> The Owner and Architect agree as follows : <br /> To design a Conference/Meeting room in the former Supervisor of Elections space in Building 2 . The subject <br /> space (approximately 1185 square feet in size) is to be reconfigured as necessary to provide a single <br /> conference room area. The subject space had been partially completed when the decision was made to <br /> proceed with this work . Limited improvements are required . Architectural and MEP engineering services <br /> shall be provided as necessary to comply with applicable Regulation/Code requirements resulting in <br /> approved construction documents suitable for construction. <br /> Schedule <br /> All preparation of Construction Documents is complete and has been transmitted to Turner Construction for <br /> pricing. <br /> Other Requirements <br /> All elements or combination of elements of the design shall conform to Applicable Regulatory requirements <br /> of Agencies having jurisdiction over the Project as a whole or in part. <br /> COMPENSATION <br /> The Architects compensation for all services under the Primary Contract including Amendments 1 through 3 <br /> is currently $2,361 ,800 (Two Million, Three Hundred Sixty One Thousand, Eight Hundred Dollars and <br /> No Cents <br /> The total increase in professional fees for this Amendment Number 4 is $8,500 (Eight Thousand, Five <br /> Hundred and No cents). This amount is to be paid from design funds budgeted for reimbursable design <br /> requirements within the agreement with Donadio and Associates. This action has no effect on the Guaranteed <br /> Maximum Prices (GMP) established for the construction of the facilities included under the contract (AIA <br /> Document A 121 CMc-2003 and AGC Document 565 . A Standard Form of Agreement between Owner and <br /> Construction Manager where the Construction Manager is also the Constructor) and it' s Exhibit (AIA <br /> Document A20M997 General Conditions to the Contract for Construction) , as approved by the Board of <br /> County Commissioners on April 19, 2005 , all as amended. <br /> The total fee amount for The Project including Amendments 1 through 4 is then $2,370,300 (Two Million, <br /> Three Hundred Seventy Thousand, Three Hundred Dollars and No Cents <br /> All Terms And Conditions Shall Remain The Same As In The "PRIMARY CONTRACT" Except As <br /> Noted Herein. <br />
