Authorization to Perform Work (APW) <br /> To : George R. Kulczycki <br /> G .K. Environmental, Inc. <br /> From: Ramie A. Gougeon, Ph.D ., RPA <br /> Panamerican Consultants, Inc. <br /> Re : A Cultural Resource Assessment Survey of the Oslo Boat Ramp Improvements <br /> Mitigation Project Area in Indian River County, Florida. <br /> This document constitutes an agreement between Panamerican Consultants, Inc. and G.K . <br /> Environmental, Inc. to perform the work described in the attached proposal . <br /> Panamerican Consultants will perform the survey of the limited areas south of Oslo Road <br /> for a fixed price of $995 .35. These prices are good for 90 days from the proposal date. <br /> After this time, prices may be altered. <br /> Panamerican Consultants will submit invoices to G.K. Environmental, Inc . , 155 McKee <br /> Lane, Vero Beach, FL, 32960, upon completion of work. After receipt of an invoice, <br /> G.K. Environmental, Inc. agrees to pay Panamerican Consultants within sixty (60) days <br /> from the invoice date. <br /> The parties to this agreement shall be responsible for maintaining all required insurance, <br /> including but not limited to, liability insurance and workman's compensation insurance in <br /> accordance with the laws of the State in which the work will be performed . <br /> The parties agree that any dispute arising between them shall be asserted and maintained <br /> in the State and County of the corporate headquarters of Panamerican Consultants, Inc. <br /> The following items are agreed to by : <br /> Panamerican Consultants, Inc.
