Indian River County, Amendment No . 1 <br /> Oslo Road Bridge Replacements and Roadway <br /> Widening <br /> • Monitor the removal of the existing structure . <br /> • Monitor pile installations . <br /> • Provide monitoring of the utility attachments to the bridges . <br /> " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : <br /> The ENGINEER will revise the Oslo Road and 43rd Avenue roadway plans such that 43rd <br /> Avenue is positioned 12 feet west of the proposed location. Work shall include revisions <br /> of 43rd Avenue plan and profile sheets , adjustment of roadway transitions, revision of <br /> signing and marking plans , revision of plateau sheet, relocation of drainage structures , <br /> elimination of guardrail where appropriate, modification of signalization plans , and <br /> adjustment of quantities . <br /> The ENGINEER will provide limited construction services assistance for structural, <br /> roadway, and utility construction elements of the Oslo Road/43rd Avenue construction <br /> project . The following provides descriptions of the tasks that the ENGINEER will <br /> perform . The general scope of work will be conducted under a lump sum budget . <br /> However, should the COUNTY request additional services other than those described <br /> below, assistance may be authorized under an hourly budget . <br /> Structural Observation Assistance <br /> • The ENGINEER will attend the Pre- Construction meeting and address the questions <br /> that pertain to the structure . <br /> • The ENGINEER will review structural shop drawings to include : <br /> 1 . concrete grout and mortar mixes <br /> 2 . precast slabs and connection details <br /> 3 . sheet piles <br /> 4 . utilities saddles and culverts <br /> 5 . resubmittals (maximum of one each) . <br /> • Observation of the bridge construction is established to provide limited observation <br /> during the installation of the structure . The ENGINEER effort will include : <br /> 1 . one site visit to observe bent cap forms and rebar placement <br /> Page 2 of 6 <br />
