Exhibit 1 - Proposal <br />Amendment No.3 <br />To <br />Work Order No. 5 <br />Scope of Services <br />Indian River County <br />Department of Utility Services <br />2008/2009 PERMIT RENEWAL <br />The current Indian River County IRCUD/West Regional WWTF Wastewater Facility - <br />Permit No. FL0041637 is expiring on March 27, 2010. According to the courtesy <br />reminder letter dated July 17, 2009, the renewal application is due on or before <br />September 27, 2009. According to the letter, failure to submit the application 180 days <br />prior to the expiration date of the referenced permit will result in enforcement action. <br />PBS&J has contacted FDEP regarding the renewal of the renewal of the permit for the <br />referenced facility and they indicated that the following forms and/or information will be <br />required <br />1. FDEP Form 1 <br />2. FDEP Form 2A <br />3. O&M Performance Report <br />4. Process Flow Diagram <br />5. Site Plan <br />6. Location Map <br />7. Binding Agreements and Documentation of Controls on Individual Users of <br />Reclaimed Water <br />8. Reclaimed Water or Effluent Analysis Report <br />9. Since the current flows to the WWTP are well below 50% of the design capacity, <br />according to rule 62-600.405(4)(C) a capacity analysis report is in not required, however <br />based on discussion with FDEP, they would require a certified statement to this effect. <br />PBS&J will address this either in the signed and sealed cover letter or in the O&M <br />performance report. <br />Based on the above PBS&J has prepared the following scope. Details of the data <br />requirements are attached in Attachment A. <br />Task 1 Proiect Kick off and Data <br />Collection <br />Task 1 requires gathering the data and updating Form 1, Form 2A and the <br />permitting package. PBSJ will work directly with the IRC and West WRF staff to obtain <br />the required data and complete the required forms and reports. A Project Kickoff <br />meeting must be held with the Staff and will be combined with the O&M performance <br />report inspection. <br />Task 2 Permitting Forms <br />Task 2 requires gathering the data and updating Form 1 and Form 2A as submitted for <br />the plant expansion. IRC will be required to complete or provide the following: <br />1. Section 3. A. Discharges To Surface Water, Items 12, 13, and 14. A detailed <br />
