The most recent Annual Reclaimed Water or Effluent Analysis Report (due <br />annually by June 28th) 7. A rnap of the monitoring well locations with GPS coordinates. <br />8. Copies of any orders or notices issued by FDEP <br />9. Copies of the Annual Flow meter Certifications for every flow meter <br />10. Digital copies of binding agreements with major reclaimed water users <br />Details of the required information are attached in Attachment A <br />Task 3 (?&M Performance Report <br />The O&M Performance report requires an inspection of the facility in the company of the <br />chief operator and an evaluation of the performance of the treatment plant. The <br />inspection will be combined with the project kick off meeting. <br />Data required include the following: <br />1. A Digital Summary of 24 months of flow and all influent and effluent data <br />covering the period from July 1, 2007 through July 30, 2009. (a digital summary of the <br />data) <br />2. A Digital Summary of the Quarterly Groundwater Monitoring Data (3`d and 4th <br />Quarter 2008, 1 st and 2" Quarter 2009) for all of the groundwater monitoring wells. <br />3. The Operating Protocol for the Facility <br />PBS&J will develop the required report and submit a draft for review prior to final <br />submittal. The owner and the chief operator will be required to sign the final document <br />prior to submission to FDEP. <br />Task 4 Response to Request for Additional Information <br />Although every effort will be made to submit a complete permitting package, this is an <br />NPDES permit and request for additional information from FDEP in response to EPA <br />overview can be expected. Should additional time an effort in excess of this fee be <br />required, PBSJ will advise the IRC prior to proceeding. <br />Submittals <br />PBSJ will provide digital copies of portions each form or report for review and comment <br />prior to assembly of the final permitting package. The final permitting package will be <br />assembled in a single bound document for signature and submittal to FDEP. The <br />submittal package will include the following: <br />1. Three copies for signature and submittal along with the permit fee of $5,000 <br />provided by IRC. The check for the permit fee should be made out to the Florida <br />Department of Environmental Regulation for renewal of IRC West WRF Plant Operating <br />Permit # FL00416374 <br />
