Catholic Charities of Diocese of Palm Beach Inc., Samaritan Center, Children 's Services Advisory Committee Grant <br /> The tools/items used are pre-post tests, school grades, school attendance, counseling & parenting <br /> class attendance and the Level system. <br /> We are not currently getting baseline information from any source on our collaborative list but <br /> will begin requesting test scores and other pertinent information from the Mental Health <br /> Association for the Child Witness Program. <br /> All activities are documented. <br /> Information is turned into our Resident Manager who inputs the statistical information. This <br /> information is kept current and entered as clients enter a new Level and as test results are <br /> completed. <br /> 3. REPORTING : What will you do with this information to show that change has <br /> occurred? How will you use or present these results to the consumer, the funder, the <br /> program, and the community? How will you use this information to improve your <br /> program? <br /> The data is collected as described in the above sections . The information becomes a permanent <br /> part of our records and is shared with those funding sources that seek that information. <br /> The "Program Beneficiary Statistical" form is the standard form that is required by Catholic <br /> Charities. This report is sent to them on a monthly basis . The outcomes are sent to Catholic <br /> Charities for review and to evaluate how our program is doing. <br /> Some of the outcomes are shared in the community when making presentations and occasionally <br /> have been used in our newsletter that is mailed to approximately 4500 area residents and <br /> businesses . <br /> We always strive to improve our program and the outcomes are used when our treatment team <br /> meets for regular evaluation of clients and the program as a whole . <br /> 13 <br />
