Catholic Charities of Diocese of Palm Beach Inc. , Samaritan Center, Children's Services Advisory Committee Grant <br /> G. TIMETABLE (Section G not to exceed one page) <br /> 1 . List the major action steps, activities or cycles of events that will occur within the <br /> program year. New programs should include any start-up planning that may occur <br /> outside the funding year. In completing the timetable, review information detailed in <br /> prior sections. <br /> Month/Period Activities <br /> Ongoing The Treatment Team consisting of Counselor, Administrator, Resident <br /> Manager, Case Manager & Children' s Coordinator meet on a weekly <br /> basis to evaluate the progress of each family. Additional focus has been <br /> placed on deadlines for achievements such as employment, educational <br /> goals, opening savings accounts, paying off old debt, etc . <br /> In addition, this team holds quarterly reviews of case files and forms to <br /> determine if improvements are needed. Findings are addressed and <br /> implemented with follow-up to determine usefulness of change . <br /> By 9/ 15/03 While we have expanded our educational video library for the <br /> Samaritan Center Children, we would like to continue expanding by <br /> increasing topics and opportunities. <br /> Ongoing Aftercare activities are vital to the continued independence and success <br /> of our former families. Regular contacts can eliminate a family sliding <br /> backwards once they have left our program. A minimum of two <br /> contacts per month will be offered to all families leaving the Center. <br /> Monthly Guest speakers from the community will be invited to speak to our <br /> residents regarding topics that will benefit them with their day to day <br /> living. These are professionals from the community who will be invited <br /> to come to the Center. This will be in addition to the mandatory weekly <br /> counseling, parenting and community meetings . <br /> 14 <br />
