SECTION 12 <br /> PROCEDURES FOR SUBMITTAL, PERMITTING , CONSTRUCTION , <br /> AND ACCEPTANCE OF PRIVATE DEVELOPMENT PROJECTS <br /> 12 . 01 SUBMITTAL <br /> A. The applicant shall submit the following items for a project to be considered <br /> for a utility construction permit: <br /> 1 . Completed Utility permit application , copy of Department of <br /> Environmental Protection ( FDEP ) permit applications if applicable , <br /> copy of Department of Transportation ( FDOT) permit application , <br /> Indian River County Right-of-Way permit, legal description , and <br /> applicable fees . Prior to the issuance of utility permit, applicant shall <br /> provide proof of all fee payment (i . e . capacity charges . Line extension <br /> fee , etc) <br /> 2. Three (3 ) sets of 24"x36" construction plans , signed and sealed by a <br /> Professional Engineer registered in the State of Florida shall be <br /> submitted with application . Plans shall include a plan view of the <br /> water and/or wastewater systems with all elevations and inverts , <br /> sewer profiles (gravity and force mains ) , all appropriate details , a <br /> drainage plan with all elevations and inverts , and water line profiles <br /> where the water main crosses other utilities and as requested by <br /> IRCDUS . Water lines shall be stationed on the plans . All existing and <br /> proposed rights-of-ways , easements , and property lines shall be <br /> shown . Also , an accurate location map is to be provided . The limits <br /> of proposed dedication of the system (s ) to Indian River County shall <br /> be specified on the plans . <br /> 3 . A complete engineer's design report, signed and sealed by a <br /> Professional Engineer registered in the State of Florida , setting forth <br /> the basis of design , shall contain the following , as a minimum : <br /> a . Water distribution system : <br /> ( 1 ) Description of geographic area to be served . <br /> (2) Existing and predicted population of areas to be <br /> served . <br /> (3 ) The effect of the proposed water distribution system on <br /> existing or proposed water distribution facilities . <br /> (4) The estimated daily flow. <br /> (5 ) Fire flow calculations for all hydrants being fed by lines <br /> smaller than 8" , or as required by IRCDUS . <br /> (6 ) Description of materials to be used . <br /> (7) A preliminary cost estimate . <br /> (8 ) Any other factors which would affect design and use of <br /> the water distribution system . <br /> b . Gravity sewer system : <br /> ( 1 ) Description of geographic area to be served . <br /> 12- 1 <br />
