�iIVE/j Board of County Commissioners -7' _ <br /> A 1840 25th Street a <br /> * * Vero Beach , Florida 32960 <br /> FLOR1pP Telephone : (772) 567-8000 <br /> FAX : 772=778 -9391 <br /> November 14 , 2005 <br /> WORK ORDER No : 2 <br /> FOR PROFESSIONAL SURVEYING SERVICES AGREEMENT <br /> FOR ANNUAL SURVEYING AND MAPPING SERVICES CONTRACT <br /> IRC CONTRACT NO , 0390 <br /> WITH <br /> BURDETTE & ASSOCIATES , INC . <br /> SPECIFIC PURPOSE SURVEY FOR <br /> HERON CAY MOBILE HOME PARK <br /> Topographic surveying related to mobile home sites and site drainage <br /> Indian River County , Florida <br /> All services performed under this contract shall be supervised and certified by a licensed <br /> Surveyor and Mapper registered with the State of Florida and staff employed directly <br /> with your firm . All survey procedures and final submittals shall meet or exceed Minimum <br /> Technical Standards for Specific Purpose as set forth in Florida Statues Chapter 61G17- <br /> 6 Florida Administrative code pursuant to section 472 . 027 , Florida Statues , <br /> Indian River County bases the following specifications on minimum requirements for this <br /> particular project . If the consultant so desires to modify or "enhance" these requirements <br /> all modifications shall be submitted in writing to the County Surveyor for approval prior to <br /> proceeding with task . <br /> SCOPE OF SERVICES <br /> Project Limits/understanding : <br /> 1 . Various mobile home site/lots (29) within the park boundary as outlined in exhibit <br /> "A" attached . <br /> 2 . Various drainage features as outlined in exhibit A . <br /> 3 . It is understood proper coordination with all entities involved ( I . E . County staff , <br /> park owners , mobile home owners ) is part of the tasks and shall be sufficiently <br /> completed prior to entering/accessing any properties . <br /> 4 . It is understood county staff and/or assigns shall utilize the information for the <br /> purpose of creating a report on the current conditions verses previously permitted <br /> and/or design plans . <br /> 5 . It is understood the estimated number of survey sites ( units ) is 29 and may be <br /> subject to change as the project proceeds . All invoicing shall be as per unit and <br /> Not to Exceed basis of $478 . 00 per unit/lot survey . * If site/unit surveys fall below <br /> 20 , fees shall be subject to negotiations . <br /> 6 .
