EXHIBIT `B" <br /> INDIAN RIVER COUNTY <br /> MAINTENANCE MAP <br /> CHECKLIST <br /> All Maintenance Map projects shall adhere to Chapter 177 & 472, F.S. and Chapter 61G17, F.A.C. as <br /> minimum and be certified as such. <br /> Projects shall be prepared/submitted in the form of a certified Maintenance Map will all applicable <br /> certifications and criteria met. <br /> 1 . Cover sheet/Key sheet/Location Sketch. <br /> 2 . Supply a Box for plat recording data, area for county acceptance and/or certification on cover <br /> sheet <br /> 3 . Survey notes and report sheet (As applicable) <br /> 4 . Any notes necessary to set out the type and/or purpose of the map shall be shown. <br /> 5 . Survey Control sheet (As applicable and or necessary) <br /> 6 . Subsequent Maintenance R/W map sheets . <br /> 7 . Title Box of each sheet must contain: <br /> a. County <br /> b. State/County Road Number or common name. <br /> c . Designed, drawn and checked by (use names or initials) & date <br /> d. Scale <br /> e. Field Book References <br /> f Sheet Numbers to be in numerical order ( 1 , 2, 3, 4, etc.) NOT ( 1 , 1A, 1B, 2, etc.) <br /> 8 . Show pertinent dimensions and bearings in the following fashion as the case may be: <br /> a. Plat — (P) <br /> b . Computed — (C) <br /> c . Field — (F) <br /> d. Deed (D) <br /> 9. Table of Ownerships shall show: <br /> a. Parcel Number <br /> b. Owners Name <br /> c. Recording Data <br /> 10. Survey Control lines and references shall be established with suitable material to perpetuate said <br /> control lines . <br /> 11 . Existing adjoining, abutting, intersecting Rights-of-Ways shall be depicted with information of <br /> record, sufficiently dimensioned and tied to control lines. <br /> 12 . The maintained lines shall be depicted as to allow for re-establishment in the field. <br /> 13 . Show stations and offsets to all existing and maintained Right of Way Breaks. <br /> 14 . Show monumented baselines and all references thereto. <br /> 15 . Verify that computed distances and bearings are supported by field measurements . <br /> 16. Verify and monument significant elements of Baseline alignment shown. <br /> (i.e . curves, bearings and P.I. Stations) <br /> 17. Ensure that significant elements of all curves are shown and are correct. Also show curve data on <br /> all sheets where they appear. <br /> 18 . Begin and End R/W Project with Stations. <br /> 19 .
