Board of County Commissioners <br />1840 25th Street 04::7DU /_ Z) A3 <br />Vero Beach, Florida 32960 <br />Telephone: (772) 5674000 <br />FAX.• 772-778-9391 <br />WORK ORDER NO. 8 <br />FOR <br />PROFESSIONAL SURVEYING SERVICES AGREEMENT <br />FOR ANNUAL SURVEYING AND MAPPING SERVICES CONTRACT NO: 0390 <br />WITH <br />Masteller, Moler & Reed, Inc. <br />This Work Order No. 8 is in accordance with the existing AGREEMENT dated September 2°d <br />2003 between Masteller, Moler & Reed, Inc. (SURVEYOR) and Indian River County <br />(COUNTY). This Work Order No. 8 amends the agreement as follows: <br />SECTION I — PROJECT LIMITS <br />This Work Order No. 8 is for the SURVEYOR to provide a route survey of the Kanawha Acres <br />Subdivision from right-of-way line to right-of-way line of 10th Street (Mastodon Rd.) west of the <br />west right-of-way line of 48`h Avenue, 11th Lane (Pine Road), 49th Avenue (Lake Road) and 50th <br />Avenue (Grove Road). <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between Masteller, Moler & Reed, Inc. and Indian River County, Masteller, <br />Moler & Reed, Inc. will perform Professional Land Surveying services for the completion and <br />preparation of a route survey for Kanawha Acres Subdivision to include the following: <br />a) Establish survey baseline with monumentation at every change of direction and at a <br />maximum of 700 foot intervals as required by Florida Statute and Indian River County. <br />b) Locate and identify all monumentation for establishment of existing right of way. <br />c) Locate and identify all visible topographic features along the route including but not <br />limited to power poles, mailboxes, pavement, driveways, culverts, fences, signs, etc. <br />d) Obtain elevations along the route sufficient for engineering design purposes. <br />e) Elevations will be based on N.G.V.D. 1929 unless otherwise directed. <br />f) Prepare and provide five (5) certified copies and one (1) digital file of the route survey <br />upon completion. <br />SECTION III — WORK PRODUCT <br />a) The SURVEYOR shall prepare its Work Product so that the COUNTY or other <br />consultants can readily use it for the design and analysis of the area, as defined. It shall <br />contain all information necessary for third -party surveyor to independently recreate <br />and/or utilize the survey work. It is acknowledged all final products become property of <br />Indian River County and will be available for use by the public at large. <br />
