ASSUMPTIONS <br />■ This Scope of Services and cost proposal is based on solid waste operations Permit Nos. SC31- <br />0128769-019 and S031-0128769-020 and the groundwater monitor wells, leachate sampling <br />sites and surface water monitor sites that are installed as of the date of this work order. <br />■ CDM Smith understands that additional new or replacement groundwater monitor wells will be <br />installed in the future under Permit Nos SC31-0128769-019 and S031-0128769-020. Sampling <br />of proposed new or replacement groundwater monitor wells required by Permit Nos. SC31- <br />0128769-019 and S031-0128769-020 beyond those currently installed and monitored (41 in <br />January and 25 in July) are not included this Scope of Services. <br />■ Assistance in revising the MIPS is not included in this Scope of Services. <br />■ Laboratory analysis is not included in this Scope of Services. <br />■ This Scope of Services and cost proposal is based on the Title V Permit No. 0610015 -004 -AV, <br />which is the current permit as of the date of this work order. <br />■ Tasks 1 through 3 do not include meetings with the Florida Department of Environmental <br />Protection. <br />DATA OR ASSISTANCE TO BE PROVIDED BY IRC SWDD <br />■ Existing data available on construction of the existing groundwater monitor wells <br />■ Available site surveys <br />■ Available record information <br />■ Laboratory analytical reports and direct access to laboratory personnel <br />■ Access to sampling site <br />■ Annual operations data needed for emissions estimating including, but not limited to tonnages <br />of waste accepted, quantity of landfill gas collected, and hours of emergency engine operation <br />■ Annual Emissions fee <br />PROJECT SCHEDULE <br />The following project schedule has been developed based on receiving authorization on before <br />February 11, 2014. <br />Task 1: January 1, 2014 through December 31, 2014 <br />Task 2: March 2014 and September 2014 (Includes sampling events, review and validation of <br />results, and completion/submittal of reports within 60 days of receipt of laboratory <br />analysis) <br />Task 3: Statement of Compliance <br />Annual Operating Report <br />Annual Emissions Fee <br />Visible Emissions Test <br />March 1, 2014 <br />April 1, 2014 <br />April 1, 2014 <br />May 1, 2014 <br />Task 4: January 1, 2014 through December 31, 2014 <br />A-5 <br />JJ1375.docx <br />
