T 13 - 10 <br /> SWE . <br /> 4010 169 <br /> CONTRACT FOR EAP SERVICES <br /> Whereas Indian River County Government , herein referred to as Customer, desires to furnish its <br /> employees and their families with a comprehensive employee assistance program ( EAP) and <br /> Corporate Care Works , Inc . herein referred to as CCW , desires to provide these services , the <br /> undersigned parties do hereby mutually agree to the following : <br /> 1 . CORPORATE CARE WORKS , INC , HEREIN REFERRED TO AS CCW, WILL BE <br /> RESPONSIBLE UNDER THIS CONTRACT : <br /> a . To provide in - person EAP counseling sessions up to 6 in - person sessions per problem <br /> situation per year for each employee and eligible family members . Licensed counselors <br /> will provide these sessions by conducting an EAP assessment , short-term counseling , <br /> and/or linkage to community resources based on the employee or their family members ' <br /> needs and counselor recommendations . These services will be provided within a 30- <br /> miles drive from employee ' s work or home location where possible . CCW EAP will assist <br /> with stress , family problems , conflicts , alcohol/drug problems , and any other personal <br /> concern that can adversely affect productivity . CCW will provide referral to appropriate <br /> childcare , eldercare , legal , financial and community services for each employee and <br /> eligible family members requesting such assistance . Telephonic and web- based <br /> services are included . <br /> b . Referrals for ongoing services needed will be the employee/family member' s <br /> responsibility and will always be at a preferred discounted rate , covered through health <br /> insurance , based on what the employee/family member can afford to pay , or free based <br /> on the community resource that counselor determines to be the best match to the <br /> client ' s needs for problem resolution . <br /> c . To provide EAP policy and recommended procedures for program operation . <br /> d . To design and provide EAP training modules for supervisors and managers in addition to <br /> employee orientations . Additionally , CCW will conduct twelve ( 12) one- hour wellness <br /> seminars . <br /> e . To provide initial and ongoing promotional materials and publicity items e . g . posters , cards , <br /> brochures , etc . , and web based EAP awareness . <br /> f. To provide unlimited consultation to Customer regarding maximizing their return on their <br /> EAP investment . <br /> g . To provide unlimited consultation and coaching to Customer' s management and human <br /> resources staff to assist management in making effective referrals to EAP services , <br /> strategizing with management regarding recommended best practice approaches , and <br /> promoting optimal productivity and performance of employees . <br />
