To provide statistical EAP Impact/Utilization Reports both quarterly and annually . <br /> j . To provide immediate crisis management consultation and response plan that includes <br /> Crisis Responder mobilization for same day onsite group debriefing services . These <br /> debriefings are typically 1 - hour in length and will be provided by CCW per incident . There <br /> is no limit to how many incidents to which CCW will respond . <br /> k . Includes mobilizing responders for same day onsite services based on customer need . <br /> I . To provide online services which include articles and resources in the areas of health , legal , <br /> financial , personal growth/online training and other tools to promote employee work/life <br /> balance . <br /> 2 . CUSTOMER WILL BE RESPONSIBLE UNDER THIS CONTRACT : <br /> a . To appoint an individual to coordinate program activities between CCW and Customer. <br /> b . To provide feedback concerning program operation . <br /> c . To coordinate benefit information with CCW staff. <br /> 3 . CCW SHALL BE COMPENSATED FOR THE AFOREMENTIONED DESCRIBED EAP <br /> SERVICES AS FOLLOWS : <br /> PER EMPLOYEE <br /> Rate : $ 1 . 65 per employee , per month <br /> Current Employee Count BOCC : 11118 <br /> Current Employee Count Sheriff' s Office : 533 <br /> Total Employee Count : 1651 <br /> Terms : Payable net 30 days from date of invoice <br /> ADDITIONAL SERVICES <br /> For additional training , onsite counseling and/or services outside the scope of EAP services <br /> described herein , CCW will be compensated at the rate of $225/hour, payable net 30 days from <br /> date of invoice . <br />
