Mr. Mike Nixon Geosyntec 30 November 2009 consultants Page 5 Task 4 — Site Assessment Addendum Report Geosyntec will prepare a brief SARA after the installation and sampling of the monitoring wells. It is anticipated that the SARA will include the following: • brief description of field activities; • results of the well survey; • general lithologic characterization; • summary tables of the groundwater sampling results and groundwater elevations; • maps that show the locations of the monitoring well network, provide a summary of the groundwater sampling results and provide a summary of the groundwater elevations; and • recommendations. A draft copy of the report will be provided to IRC for review. Any comments from IRC will be incorporated into the SARA and a final version of the report will be prepared. One hard copy and one electronic copy of the final report will be submitted to the FDEP for review and two hard copies of the fmal report will be submitted to IRC. The final reports will be signed/sealed by the Florida Licensed Professional Engineer in responsible charge of the project. A breakdown of estimated project costs is included as Attachment B. Task 5 — Semi -Annual Monitoring Well Sampling Task 5 is included to cover one semi-annual sampling event for the monitoring well network, which will likely be required by the FDEP, assuming a Natural Attenuation Monitoring program is approved for the site by the FDEP. This event would occur after completion and submittal of the SARA to the FDEP and approximately six months after the monitoring well network was sampled previously existing wells and This event includes the sampling of the monitoring well network (three six newly installed wells). It also includes collecting a set of water level measurements from the monitoring well network. The monitoring wells will be sampled and purged according to the FDEP SOPS and the groundwater samples collected will be laboratory analyzed for arsenic by EPA Method 6010. Task 6 — Semi -Annual Sampling Letter Report Geosyntec will prepare a Semi -Annual Sampling Letter Report after completion of the semi- annual monitoring well sampling event.
