Mr. Mike Nixon Geosyntec <br />30 November 2009 consultants <br />Page 5 <br />Task 4 — Site Assessment Addendum Report <br />Geosyntec will prepare a brief SARA after the installation and sampling of the monitoring wells. <br />It is anticipated that the SARA will include the following: <br />• brief description of field activities; <br />• results of the well survey; <br />• general lithologic characterization; <br />• summary tables of the groundwater sampling results and groundwater elevations; <br />• maps that show the locations of the monitoring well network, provide a summary of the <br />groundwater sampling results and provide a summary of the groundwater elevations; and <br />• recommendations. <br />A draft copy of the report will be provided to IRC for review. Any comments from IRC will be <br />incorporated into the SARA and a final version of the report will be prepared. One hard copy <br />and one electronic copy of the final report will be submitted to the FDEP for review and two <br />hard copies of the fmal report will be submitted to IRC. The final reports will be signed/sealed <br />by the Florida Licensed Professional Engineer in responsible charge of the project. <br />A breakdown of estimated project costs is included as Attachment B. <br />Task 5 — Semi -Annual Monitoring Well Sampling <br />Task 5 is included to cover one semi-annual sampling event for the monitoring well network, <br />which will likely be required by the FDEP, assuming a Natural Attenuation Monitoring program <br />is approved for the site by the FDEP. This event would occur after completion and submittal of <br />the SARA to the FDEP and approximately six months after the monitoring well network was <br />sampled previously <br />existing wells and <br />This event includes the sampling of the monitoring well network (three <br />six newly installed wells). It also includes collecting a set of water level <br />measurements from the monitoring well network. The monitoring wells will be sampled and <br />purged according to the FDEP SOPS and the groundwater samples collected will be laboratory <br />analyzed for arsenic by EPA Method 6010. <br />Task 6 — Semi -Annual Sampling Letter Report <br />Geosyntec will prepare a Semi -Annual Sampling Letter Report after completion of the semi- <br />annual monitoring well sampling event.
