EXHIBIT "A' <br />AGREEMENT FOR PROFESSIONAL SERVICES <br />Work Order No. 1 <br />(Engineering/Survey) Continuing Professional Surveying and Mapping/GIS Services Contract #1333 <br />Indian River County Project #1362 <br />PROJECT: Old Dixie Highway/Highlands Drive <br />CLIENT NAME Indian River County — David Schryver <br />SITE Highlands Drive and OId Dixie Highway <br />ADDRESS: Vero Beach, FL 32968 <br />PHONE: Work - (772) 226-1386 <br />PHONE: Fax - (772) 778-9391 <br />MAILING 1801 27th Street <br />ADDRESS: Vero Beach, FL 32960 <br />Highlands Drive Right of Way and Topographic Survey <br />Pursuant to our Annual Land Surveying and Mapping/GIS Services Contract 1333, MMRT, Inc. is <br />pleased to provide Indian River County with this proposal for performance of a Specific Purpose Survey <br />to establish Right of Way and perform a Topographic Survey for property located in Indian River County <br />and Tying in Section 31, Township 033 South, Range 040 East which shall include the following: <br />Proiect Outline and Limits: <br />The Survey limits are generally described as the Highlands Drive Right of Way from the F E C Railroad <br />to 450 feet west of OId Dixie Highway including 100 feet each way on side streets and the FEC railroad. <br />The survey limits shall also extend 1100 feet north and south of Highlands Dnve along Old Dixie <br />Highway. The approximate length of the right of way limits is 3,440 linear feet (0.63 miles). <br />Scope of Services: <br />Task 1: Right of Way and Topographic Survey <br />1) Project initiation, including review of existing deeds and surveys, files and field notes, preparation of <br />work orders and field crew folders. <br />2) Find, recover and measure field ties into existing survey control along the route to establish the <br />rights of way and baseline of survey. Tie Horizontal Control to Florida State Plane Coordinate <br />System, Florida East Zone, NAD 83. <br />3) Find and recover existing vertical control. Elevations shall be based upon the North American <br />Vertical Datum of 1988 (NAVD 88) with a conversion to the National Geodetic Vertical Datum of <br />1929 (NGVD 29) provided. Perform bench run and establish benchmarks along the survey corridor <br />at maximum 600 foot intervals. <br />4) Field monument baseline control at maximum 600 foot intervals. Control points shall consist of iron <br />rod and cap or nail and disk. Control points shall be designated on survey with station/offset and <br />NAVD '88 elevation. <br />5) Field locate and record vertical ground elevation cross sections within and 10' outside of right of <br />way at even 50 foot intervals and all distinct locations (inverts retention areas, swales, ditches etc.) <br />and changes in contour along the right of way corridor. <br />6) Field locate and record existing improvements and man made features within and 10' outside of <br />right of way, including but not limited to, roads, swales, driveways, culverts, signage and mailboxes, <br />visible utilities fences etc within and 10' outside the existing Right of Way. <br />7) Field locate and identify limits of vegetation and trees, 4" or larger within and 10 feet outside the <br />right of way limits. <br />8) Process field data and determine limits of existing Highlands Drive OId Dixie Highway and FEC <br />Railroad Rights of Way. Prepare survey drawing in accordance with Minimum Technical Standards <br />including cover sheet with location sketch, certifications related title and project number, notes, <br />legend, abbreviations and plan view sheets.
