EXHIBIT "A" <br />AGREEMENT FOR PROFESSIONAL SERVICES <br />Work Order No. 1 <br />(Engineering/Survey) Continuing Professional Surveying and Mapping/GIS Services Contract #1333 <br />Indian River County Project #1362 <br />Time of Completion and Deliverables: <br />1. Project shall be completed as follows: <br />a. 100 % "Paper" review submittal (final review prior to final deliverables) shall be made within <br />45 calendar days of receipt of Notice to Proceed for review by County Staff prior to <br />preparing final submittal package. <br />b. Time of Final project completion shall be within 10 calendar days of receipt of the County's <br />review comments from the 100% paper submittal <br />2. Deliverables -The SURVEYOR shall provide the COUNTY: <br />a. 100% Phase Submittal for County review and comment All submittals shall include one (1) <br />paper 'hardcopy" along with Autocad files and an ASCII file for all survey points. County <br />shall strive to provide review comments within 10 calendar days of the preliminary submittal. <br />b. FINAL Deliverables, Five (5) paper "hardcopy" signed and sealed sets and one (1) signed <br />and sealed MYLAR set in adequate recording fashion (I E Borders). Sheeted and model <br />space (as applicable) AutoCAD drawing file in release 2004 through 2009, PDF formats and <br />an ASCII file for all survey points, all electronic files to be on CD. <br />c. Survey set/sheets shall include a cover sheet with location sketch, survey certifications, and <br />related title and project number; survey notes, legend and abbreviations and plan view <br />sheets. <br />d. Work product and digital versions are to be prepared and submitted so that the County or <br />other consultants can readily use it for design and analysis of the area as defined. It shall <br />contain all information necessary for a third party surveyor to independently recreate or <br />utilize the survey work. It is acknowledged all final products become property of Indian <br />River County and will be available for use by the public at large <br />e. The SURVEYOR s work product shall meet or exceed the minimum standards defined by <br />Sections ll, III and IV or the COUNTY will not approve the SURVEYOR's request for <br />payment. <br />Compensation: <br />Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services <br />Agreement, Contract No. 1333 with the Indian River County <br />I. Project initiation, including review of existing deeds and surveys, files and field notes, <br />preparation of work orders and field crew folders. Find, recover and measure field ties into <br />existing survey control along the route to establish the rights of way and baseline of survey. Tie <br />Horizontal Control to the Florida State Plane Coordinate System, Florida East Zone, NAD 83. <br />Find and recover existing vertical control. Elevations shall be based upon the North American <br />Vertical Datum of 1988 (NAVD 88) with a conversion to the National Geodetic Vertical Datum of <br />1929 (NGVD 29) provided. Perform bench run and establish benchmarks along the survey <br />corridor at maximum 600 foot intervals. Field monument baseline control at maximum 600 <br />intervals Control points shall consist of iron rod and cap or nail and disk.
