EXHIBIT " A " <br /> AGREEMENT FOR PROFESSIONAL SERVICES <br /> Work Order No . 2 <br /> ( Engineering/Survey) Continuing Professional Surveying and Mapping/GIS Services Contract # 1032 <br /> Indian River County Project # 0934A <br /> 8 ) Field locate and obtain west invert and structure size of all U . S . Highway 1 cross drains via <br /> drainage outfalls , ( swales , ditches canals etc . ) . Tie to existing baseline as appropriate . <br /> 9 ) Process field data and prepare Topographic survey in accordance with Minimum Technical <br /> Standards . Survey shall show Old Dixie right of way and baseline and related data . Prepare survey <br /> set sheets , including cover sheet with location sketch , certifications , related title and project <br /> number, notes , legend , abbreviations and plan view sheets . Prepare final deliverables . <br /> Notes : <br /> County to provided one hard copy and AutoCad digital file of Right of Way Map for Old Dixie Highway <br /> within 10 days of notice to proceed . Phase submittals shall be provided for billing support . <br /> Time of Completion and Deliverables : <br /> 10 ) Deliverables : <br /> a ) Final review shall include one 100 % " Paper" copy be submitted prior to the Final Deliverables <br /> for the project to ensure all previous comments have been addressed . <br /> b ) FINAL — Five ( 5 ) paper " hardcopy" signed and sealed set and one ( 1 ) signed and <br /> sealed <br /> MYLAR set in adequate recording fashion ( I . E . Borders ) . Sheeted and model space ( as <br /> applicable ) AutoCAD drawing file and PDF formats on CD . <br /> • Survey set/sheets shall include a cover sheet with location sketch , survey certifications , and <br /> related title and project number; survey notes/legend/abbreviation sheet , and right-of-way <br /> plan view sheets . <br /> Compensation . <br /> Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services <br /> Agreement , Contract No . 1032 with the Indian River County. <br /> I . Project initiation , including review of existing deeds and surveys , files and field <br /> notes , preparation of work orders and field crew folders . Find , recover and tie into <br /> existing survey control based upon existing Right of Way map as prepared by Carter <br /> & Associates to be provided by the County. Horizontal Control shall be based upon <br /> the coordinate system on said existing Right of Way map . Find and recover existing <br /> vertical control . Elevations hall be based upon the National Geodetic Vertical Datum <br /> of 1929 ( NGVD 29) with a conversion to the North American Vertical Datum of 1988 <br /> ( NAVD 88) provided .
