4 . Issuance of all permits and certificates , inspections , and other approvals and releases by <br /> governing authorities required for Owner' s occupancy and use of the project. <br /> 5 . Project site shall be cleared of Contractor' s excess equipment and/or supplies and <br /> material . <br /> 6 . All sets of operation and maintenance manuals for all equipment shall have been <br /> submitted , as referenced in the technical specifications . <br /> 7 . Manufacturers' certifications and warranties shall be delivered to Owner. <br /> 8 . All operations and maintenance manuals for all equipment shall be delivered to Owner. <br /> 9 . All required spare parts , as well as any special devises and tools shall be delivered to <br /> Owner. <br /> 10 . All keys and blanks shall have been provided . <br /> 11 . Punch list of items required to render the construction services complete , satisfactory and <br /> acceptable to the Owner. If the Contract provides for a multi -phased or multi-structure <br /> project, a punch list must be developed for each phase or structure . <br /> Contractor shall arrange a joint site and project inspection ( s) with Engineer for inspection for <br /> substantial completion and development of a punch list for the completion of construction services . Said <br /> punch list shall be prepared not later than thirty ( 30) days after the inspection for substantial completion . <br /> Punch list items recorded as a result of inspections for substantial completion shall be corrected by the <br /> Contractor prior to any request for final inspection and acceptance . <br /> If substantial completion is not obtained at the inspection called by Contractor, for reasons which <br /> are the fault of Contractor, any additional cost to Owner for the Engineer or design professional for any <br /> subsequent inspections for the purpose of determining substantial completion shall be the responsibility <br /> of the Contractor and shall be assessed against the final payment application . <br /> GC 41 FINAL INSPECTION AND ACCEPTANCE <br /> The following items shall be completed as a condition precedent to a request by Contractor for <br /> final inspection : <br /> 1 . Completion of all punch list items recorded from the substantial completion inspection . <br /> 2 . A complete set of mylar or paper marked -up as- builts with " RECORD" or " AS BUILT" <br /> clearly printed on each sheet along with one copy on AutoCAD CD Release 14 or better on State Plane <br /> Coordinates . Contractor shall accurately and neatly transfer all deviations from progress as-builts to final <br /> as-builts . As-built records shall include drawings signed and sealed by a registered land surveyor <br /> certifying the elevation and location of improvements . Certified drawings are to show locations , lines and <br /> grade of buried pipe line four (4 ) inches or larger in diameter and exterior to buildings , and other buried <br /> facilities (e . g . valves , tanks , vaults , storm inlets , ducts , etc. ) installed or discovered as a result of the work . <br /> Certified drawings are to show building corners , sidewalks , paved areas and the location of all above <br /> ground structures within the project site . Contractor shall sign each final record drawing and note thereon <br /> that the final as- builts are complete and accurate . <br /> 3 . A complete set of marked -up specifications with " RECORD" or "AS-BUILT" clearly printed <br /> on the cover. Contractor shall accurately and neatly transfer all deviations from progress specifications to <br /> final as-builts . <br /> 4 . A signed and notarized affidavit indicating that no asbestos containing materials were <br /> used or installed during the course of the work . <br /> 5 . A list of each piece of equipment having an individual value of $ 500 or greater. The list <br /> shall include , at a minimum , the name , make and model number, quantity installed , value of equipment. <br /> Contractor shall inform Owner in writing that work is ready for final inspection . The Engineer will , <br /> within five ( 5) business days , make such inspection . When Engineer finds the work acceptable under the <br /> Contract and all contract terms and conditions fully performed , Engineer shall issue a Final Certification to <br /> Owner. <br /> GC 42 EXTENSION OF TIME . <br /> The time limit for the completion of the work as described in the Contract is of the essence of this <br /> Contract. Contractor must request an extension of the contract completion date in writing and must <br /> 41 <br /> 12/1!2009 3:45:46 PM <br />
