ORDINANCE 96- 0 3 <br />Section 304.16 is hereby amended as follows <br />Section 304.16. Record Keeping. <br />a. Any agency providing transportation under Certificate <br />"E" of this chapter STM shall be under contract with the community <br />transportation coordinator. Said contract PM shall set forth the <br />specific record keeping requirement. All records shall be kept for <br />a minimum of five (5) years. Said records shall be available for <br />inspection by a representative of the county. <br />bo Any agency providing transportation under Certificate <br />"E1" of this chapter shall maintain a record of trips provided <br />and such other related information as the EMS Director <br />requires . Said records shall be sent to the EMS Director <br />periodically when requested. No further record retention i <br />us <br />required . <br />Section 304.17 is hereby amended as follows <br />Section 304.17. Community transportation <br />"' coordinator. <br />All services subject to )at`/ Certificate "E" of this chapter <br />ib� /�' / ih� / 161` / �' / ihli / �vYi�Y / �i'YY� /���i►�ni <br />fihfid$ shall be under written contract with the community <br />transportation coordinator and shall be coordinated through <br />the Indian River County Community Transportation <br />Coordinator. <br />SECTION 2. SEVERABILITY. <br />If any section, or any sentence, paragraph, phrase, or word of <br />this ordinance is for any reason held to be unconstitutional, <br />inoperative, or void, such holding shall not affect the remaining <br />legislative intent to pass the ordinance without such unconstitutional, <br />invalid or inoperative part. <br />SECTION 3. EFFECTIVE DATE. <br />A certified copy of this ordinance, as enacted, shall be filed by <br />the Clerk with the Office of the Secretary of State of the State of <br />Words indicated as �jk/f4t0jht4 are deletions <br />from existing law, words indicated as underlined are additions. <br />
