AMERICANS WITH DISABILITIES ACT (ADA) <br />COMPLAINT RESOLUTION PROCEDURE <br />1) If a member of the general public believes that they have been <br />discriminated against on the basis of a disability by the County, that <br />person or his authorized representative may submit a written complaint to <br />the County's ADA Coordinator at 1840 25th Street, Vero Beach, Florida <br />32960. The written complaint must include the following four items: 1 ) <br />the name and address of the individual or authorized representative filing <br />the complaint; 2) a detailed description of the county's alleged <br />discriminatory action including the nature and date of the alleged violation; <br />3) a signature of the complaintant or the authorized representatives 4) a <br />description or identification (by name, if possible) of alleged victims of <br />discrimination if the complaint is filed on behalf of a class of individuals. <br />The written complaint must also be submitted within 180 days from the date <br />of the alleged discrimination unless a good cause for the delay can be <br />demonstrated to the ADA Coordinator. <br />After the complaint has been received, the ADA Coordinator will review the <br />complaint and attempt to resolve it promptly and equitably. The <br />Coordinator will respond to the complainant in writing within ten business <br />days from the date the complaint was received by the Coordinator with the <br />action to be taken . <br />2) If the complaint cannot be resolved to the satisfaction of the complainant by <br />the ADA Coordinator, the complainant may forward the written complaint to <br />the County's ADA Compliance Committee at 1840 25th Street, Vero Beach, <br />Florida 32960 within five business days of receiving the Coordinator's <br />written response. This informal County committee is composed of the ADA <br />Coordinator, Personnel Director, Deputy County Attorney, Buildings and <br />Grounds Superintendent, Building Department Director, and a member of <br />the disabled community, or their designees. This committee will review the <br />complaint and attempt to resolve it promptly and equitably. The Committee <br />wilt respond to the complainant in writing within ten business days from the <br />date the complaint was received by the Committee with the action to be <br />taken. <br />3) If the complaint cannot be resolved to the satisfaction of the complainant by <br />the ADA Compliance Committee, the complainant may forward the written <br />complaint to the County Administrator at 1840 25th Street, Vero Beach, <br />Florida 32960 within five business days of receiving the Compliance <br />Committee's written response. The Administrator will review the complaint <br />and attempt to resolve it promptly and equitably. The Administrator will <br />respond to the complainant in writing within ten business days from the <br />date the complaint was received by the Administrator with the action to be <br />taken . <br />4) If the complaint cannot be resolved to the satisfaction of the complainant by <br />the County Administrator, the complainant may forward the written <br />complaint to the County Administrator's office at 1840 25th Street, Vero <br />• �. •"nX -3G <br />
