ORDINANCE NO. 2002-031 <br />the project <br />design <br />employs <br />each of these <br />attributes <br />in a manner <br />that <br />minimizes <br />adverse effects <br />to <br />the greatest <br />extent <br />feasible to <br />achieve <br />the wireless <br />service <br />capabilities <br />demonstrated <br />to be <br />necessary <br />under <br />subsection <br />5(a)(1)n. <br />(2) Submittal requirements for new non -stealth antenna supporting <br />structure applications: <br />a. A completed application form and any appropriate fees; and <br />b. Three (3) sets of signed and sealed site plans; and <br />c. A property card for the subject property from the Indian River <br />County Property Appraiser's Office or a tax bill showing the <br />ownership of the subject parcel; and <br />d. A form indicating that a property and/or antenna -supporting <br />structure's owner's agent has authorization to act upon their <br />behalf (if applicable); and <br />e. A signed statement from the antenna -supporting structure's <br />owner or owner's agent stating that the radio frequency <br />emissions comply with FCC standards for such emissions; and <br />f. Proof of an FCC license to transmit and/or receive radio signals <br />in Indian River County; and <br />g. Prior to issuance of a building permit, a stamped or sealed <br />structural analysis of the proposed antenna -supporting structure <br />prepared by a licensed Florida engineer indicating the proposed <br />and future loading capacity of the antenna -supporting structure; <br />and <br />h. One original and two (2) copies of a survey of the property <br />completed by a licensed Florida engineer which shows all <br />existing uses, structures, and improvements; and <br />i. Three (3) copies of a vegetation survey or Habitat Evaluation <br />Index (HEI); and <br />j. Photo -simulated p ost c onstruction r enderings o f t he p roposed <br />antenna -supporting structure, equipment enclosures, and <br />ancillary structures as they would look after construction from <br />locations to be determined during the pre -application <br />conference ( but shall, at a minimum include renderings from <br />Coding: Words in stfilce thr'" ug are deletions from existing law.
