within the following twelve (12) months. Failure to <br />either: <br />a. make application for, and within twelve (12) <br />months of application obtain, final plat <br />approval of a development phase or <br />b. to request, and within three (3) months of the <br />request obtain, the issuance of a certificate <br />of completion for a development phase on an <br />approved preliminary plat within a period of <br />eighteen (18) months from the date of approval <br />of the preliminary plat constitutes as <br />expiration of the plat unless the applicant <br />applies and receives approval for an extension <br />pursuant to 913.07(4)(K), or extension of a <br />phase or phases of a project pursuant to <br />913.07(4)(J)2. <br />4. Any phase of development not constructed within the <br />approved timeframes will result in expiration of the <br />preliminary plat. The applicant must obtain re - <br />approval of the phase schedule prior to the <br />expiration date of the phase. If the applicant does <br />not obtain re -approval of the phase schedule, the <br />preliminary plat will be subject to further <br />conditions by the, board of county commissioners or <br />be terminated. <br />(B) Procedure. No final plat application shall be submitted <br />for approval prior to the issuance of land development <br />permit. The plat shall be accompanied by: <br />1. A complete final plat application as furnished <br />by the planning department; <br />2. The appropriate filing fee and application; <br />3. A certificate of concurrency covering the area <br />to be platted; <br />4. When required in conjunction with a <br />construction contract or maintenance agreement, <br />a certified cost estimate shall be prepared by <br />the developer's engineer and shall include the <br />cost of surveying, engineering and construction <br />of all required improvements in substantially <br />the following form: <br />300 <br />
