detailed in drawing No. 6 section 312.19. Density testing shall be performed by <br />an approved independent testing laboratory and reports submitted to the Public <br />Works Director prior to permanent patch. A temporary asphalt patch shall be <br />applied within twenty-four (24) hours after backfill and compaction. The finish <br />surface of the temporary asphalt patch shall be within one-quarter (114) inch of <br />the elevation of the existing roadway surface. <br />2) Seventy-two (72) hours prior to the time of pavement cut, the Public Works <br />Director shall be notified of the date and time of the cut. An emergency phone <br />number of the Contractor shall be provided. <br />3) Work site traffic control shall be provided in conformance with "Florida <br />Department of Transportation Roadway and Traffic Design Standards." <br />4) Asphalt restoration will be required within fifteen (15) days after temporary patch <br />is installed. The final finish shall be smooth and uniform within a one-quarter (114) <br />inch tolerance of the required surface. <br />5) Shoulder, right-of-way sodding, and other restoration shall be performed as <br />required by the Public Works Director in accordance with Florida Department of <br />Transportation Standards. <br />6) Traffic control devices (e.g. signs, street name signs, traffic signals and pavement <br />markings) shall be restored to their original condition or better. <br />7) Prior to final release of the performance bond, which shall occur no sooner than <br />ninety (90) days after final pavement restoration, the Engineer -of -record or utility <br />company shall request a final inspection, shali certify that the utility has been <br />installed in accordance with standards specified in the publications listed in <br />section 312.08, shall submit two (2) copies of as -built drawings, and shall submit <br />release of lien by Contractors. <br />8) All utility structures will be required to have an identification tag designating <br />proper ownership. <br />Restoration <br />All surfaces, as described, shall be completed as soon as is reasonable. In no case shall the <br />pavement or driveway replacement operation be unfinished for more than 10 days after backfilling <br />unless otherwise directed by the Engineer. Replace all damaged or cut pavement due to Contractor's <br />operations; restore all pavement outside of trench area that is damaged by the Contract at no <br />expense to Owner. <br />Guarantee <br />All restored areas within the public right-of-way shall be guaranteed for one year. In the event of <br />settlement of paved areas more than one-quarter inch below the undisturbed adjacent permanent <br />pavement, the Contractor shall make the necessary repairs to restore the pavement level within ten <br />calendar days after notification by the Owner.
