12.2.3 Documentation and Follow -Up <br />During all phases of response, documentation should be collected <br />and maintained to support all actions taken under this Plan, and to <br />form the basis for cost recovery. In general, documentation should <br />be sufficient to provide the source and circumstances of the <br />condition, the identity of responsible parties, accurate accounting of <br />local or private party costs incurred, and impacts and potential <br />impacts to the public health, welfare and the environment. <br />Evidentiary and cost documentation procedures and requirements <br />to be followed will be those specified in the USCG Marine Safety <br />Manual (Commandant Instruction M 16000.3) and 33 CFR Part <br />153. <br />A final report of the incident should be prepared by the lead <br />response agency which includes, at a minimum, the following <br />information: <br />- time and date of incident; <br />- name and address of affected facility; <br />- name of facility owner/operator; <br />- hazardous material(s) involved; <br />nature and source of release; <br />- summary of actions taken to protect public <br />health/safety, the environment and other property; <br />- summary of injuries and property damage; <br />documentation of costs; and <br />- need for additional actions. <br />The information and reports obtained by the lead agency for <br />response actions shall, as appropriate, be transmitted to the <br />chairman of the Local Emergency Planning Committee and the <br />chairman of the State Emergency Response Commission for <br />Hazardous Materials. <br />Indian River County Emergency Plan for Hazardous Materials Page 81 <br />
