ATTACHEMENT "A" <br />JUSTIFICATION OF CHANGE ORDER NO. 2 <br />South County Water System <br />Water System Improvements <br />Contract Part I - Distribution System <br />Indian River County, Florida <br />ITEM 1- 20th Avenue Water Line Extension <br />In order to provide improved water service to the Ixora Subdivision, it is necessary to <br />construct the 20th Avenue water line extension. These improvements will be accomplished <br />by the construction of an 8 inch water line, tying into the existing Mid -Florida system <br />at 8th Street, running south along 20th Avenue to Oslo Road, where it will tie-in to the <br />20 inch water main from the South County water treatment plant. The contractor has <br />agreed to maintain the tame unit prices as were bid for their original contract for Part <br />I - Distribution System. Two additional items must be added to the bid schedule which <br />are: 1. the bridge crossing at the South Relief Canal, and 2. remobilization and re - <br />demobilization. The item remobilization and redemobilization is justified since the <br />contractor has removed his equipment and crews from the project site. See -Attachment "B"' <br />for the detailed breakdown of the required estimated quantities and cost to:.;conztruct the <br />20th Avenue water line extension. <br />ITEM 2 - Installation of a 12 inch Gate Valve and Box <br />Indian River County is presently purchasing water from the City of Vero Beach for the <br />South County area. This water is supplied by a 12 inch water line which runs along the <br />west side of Old Dixie Highway, south from the City Limits of Vero Beach. In order to <br />isolate the new South County water system from the City of Vero Beach, it will be <br />necessary to install a 12 inch gate valve and box in the existing 12 inch water line. <br />The contractor has agreed to install this 12 inchgate valve and.box.for an additional <br />cost of $2,750.00. <br />ITEM 3 - Mid -Florida Tie -In <br />The Mid -Florida tie-in was deleted from the original contract prior to the actual award <br />of contract because the necessary easements had not yet been obtained. Indian River <br />County's Legal Department has now obtained all the required,easements so that the Mid - <br />Florida tie-in can be added back in the contractor's original contraQt. The Mid -Florid;. <br />tie-in consists of the installation of a 12 inch water ma n..connect;ing the Mid -Florida <br />elevated storage tank to an existing looped 8 inch water :;i.ne around, the Vero Niall. The <br />cost of this tie-in will be at the same unit prices as quoUd in the;contracttr t <br />original bid. <br />ON MOTION BY Commissioner Fletcher, SECONDED by <br />Commissioner Lyons, the Board unanimously agreed <br />to extend the meeting to 5:22 o'clock P.M. <br />CHANGE ORDER NO. 3, SOUTH COUNTY WATER SYSTEM - CONTRACT <br />PART 3 - GULF CONSTRUCTORS <br />Engineer John Robbins reviewed the proposed Change <br />Order, as .follows <br />3. <br />OCT 2 01992 pox 5-1 w 757 <br />
