rOCT 19 1983 <br />HES SLUDGE MANAGEMENT SYSTEMS, INC. <br />P.O. BOX 1261 <br />Boynton Beach, Florida 33425 <br />We are pleased to provide this cost effective proposal for your <br />review and consideration. E P A is a full service Sludge <br />Management Company. Our range of services include; Septage <br />Treatment Systems: Consulting, Design and Operation. <br />SCOPE OF SERVICES <br />1.. HES shall construct a treatment facility for Septage Waste <br />and operate the facility at NO cost to the County. <br />2. HES shall receive dump fees from the users of the facility <br />to cover construction and operational costs. <br />3. HES shall secure all permits for the installation and <br />operation of the treatment facility that are required by <br />law and shall bear the expense of obtaining said permits. <br />4. HES shall maintain daily supervision of the treatment <br />facility by a Certified Operator licensed by the Florida <br />Department of Environmental Regulation. <br />5. The County shall provide HES with a site for the <br />construction of the treatment facility for the fee of $ 1.00 <br />per year. The site lease shall be for at least a five (5) <br />year period. <br />6. The County shall construct and maintain a shell rock access <br />road to the treatment facility and provide an electrical <br />power source. <br />7. The County agrees to forward all septage received at the <br />landfill to the HES treatment facility. <br />8. The County agrees that HES will not accept hazardous or <br />industrial waste. <br />9. HES agrees to begin construction within fifteen (15) days <br />and begin operation within fifteen (15) days of receipt of <br />the Florida Department of Environmental Regulation permit. <br />TERM. The term of this Agreement shall be for a period of five <br />(5 years. This agreement shall be renewed automatically for an <br />additional term of five (5) years at the end of the initial term <br />unless the County notifies HES at least one (1) year prior to <br />the end of the initial term as to the County's intentions to <br />terminate such agreement: <br />36 <br />
