TO: Board of County <br />Commissioners <br />DATE: October 18, 1984 FILE: <br />THRU: Michael Wright SUBJECT: <br />County Administrator Documentation for Project 003 <br />FROM: Edward J Rega REFERENCES: <br />Executive Director <br />IRC Housing Authority <br />It is recommended that the data herein presented be given <br />formal consideration by the County Commission. <br />The Department of Housing and Urban Development in their <br />letter of September 21, 1984 approved our application for an <br />additional 24 units -and the amount of $104,940 has been re- <br />served pending the submission of required documents. When the <br />requested documents have been received and approved by HUD they <br />will execute the Annual Contributions Contract and forward to us <br />for execution. <br />The documentation required by HUD is submitted and attached <br />for consideration and approval of the Board of County Commissioners. <br />1. HUD form 916, Certifications In Connection With The <br />Operation of a Section 8 Existing Housing Program. <br />2. Revision I, Estimate of Total Required Annual Con- <br />tributions - HUD Form 52673 with two (2) HUD Form <br />52672 -. Estimate of Required Annual Contributions, <br />attached. <br />I recommend the County Commission authorize the Chairman of <br />the Board to execute forms HUD 916 and HUD 52673, Rev. I, with <br />attachments for transmittal to the Jacksonville office of HUD. <br />ON MOTION by Commissioner Bird, SECONDED <br />by Commissioner Wodtke, the Board unanimously <br />(4-0) approved Hud Forms 916, 52673, and 52672, <br />as described above, and authorized the signature <br />of the Chairman. <br />7 <br />N 0 V 7 198BOOK 58 'A "r P) j <br />
